The Effect Of Lmwh For The Change Of Coagulation And Fibrinolytic Markers In Elderly Patients With AECOPD

ObjectiveTo observe the clinical significance of LMWH for the change of coagulation and fibrinolytic markers in elderly patients with AECOPD.Evaluation of LMWH in the treatment of elderly patients with AECOPD.Methods50 elderly patients with AECOPD were randomly divided into the group of treatment and control,25 patients in each group,the control group was given conventional therapy(including anti infection,expectorant,antiasthmatic,nutritional support,correction of water and electrolyte disorders and other treatment),the treatment group was injected of LMWH 4100u,subcutaneous injection,q12h,for seven days on the basis of control group.Contrast the routine coagulation indexactivated partial thromboplastin time(APTT),prothrombin time(PT)and thrombin time(TT)and the changes of coagulation and fibrinolytic markers:the endothelin-1(ET-1),hemocysteine(HCY),D-dimer,fibrinogen and difference of indexes of blood gas analysis and treatment.Quantitative data are expressed in terms of mean standard deviation,state 14.0 software was used for statistical analysis.Results1.The changes of the routine coagulation index and coagulation and fibrinolytic markers in two groups are obvious(P<0.05),compared with the pretherapy,the treatment groop changes of the routine coagulation index and coagulation and fibrinolytic markers are more obvious after therapy,the difference between the two groups was statistically significant(P<0.05).2.The difference of blood gas analysis index in the treatment group is significantly better than that in the control group,the difference between the two groups was statistically significant(P<0.05).3.The ventilator usage time,EICU stay time and treatment cost in the treatment cost in the treatment group were significantly lower than those in the control group,the difference between the two groups was statistically significant(P<0.05).ConclusionThe level of blood coagulation markers in elderly patients with AECOPD increased significantly,and the blood showed hypercoagulable state,LMWH significantly improved the coagulation and fibrinolysis status in elderly patients with AECOPD,promoted the recovery of ventilatory function,achieved good therapeutic effect,with less adverse reactions and significant clinical effect.
