Atrial fibrillation(AF)is one of the most common and serious arrhythmias in the clinic.According to the Framingham study,the incidence of AF patient was around 0.5%,and the rate increases with age.At present,catheter ablation has become the first-line treatment for symptomatic AF patients,with the continuous promotion of this technology,its complications are increasing too,and the common complications included subcutaneous hemorrhage and hematoma,arteriovenous fistula,peripheral vascular embolism,cardiac tamponade,pulmonary vein stenosis,atrial esophageal fistula,phrenic nerve injury,etc..The main complications of digestive system include peripheral vagus nerve injury,gastrointestinal dysfunction,paralytic ileus,gastroparesis,atrial esophageal fistula and phrenic nerve injuryThe primary gastrointestinal symptoms,after radiofrequency catheter ablation of atrial fibrillation,were nausea,vomiting,retching,abdominal pain,more and more these kind of symptoms have been reported nationally and internationally.Can we use the traditional Chinese medicine to prevent them?Is it effective?There is no such study yet.The traditional Chinese medicine believes that "ears are the sources and cross point for twelve channels".Auricular point therapy can harmonize "Qi and blood stream",dredge meridians and achieve therapeutic purposes through the action of meridians.Although the mechanism of the meridian is unclear,but the auricular experiments of acupuncture found that auricular channel transmission phenomenon along meridian line,a large number of clinical practice showed that the ear and meridian relation is the really existence and points have certain specificity.The auricular point can decrease the stomach uncomfortable,restrain the spasm of diaphragm,stop vomiting,and relieve the nausea and vomiting caused by the stimulation of vagus nerve.In conclusion,there have been studies to confirm the effect of buried seeds of auricular points on gastrointestinal symptoms,but there is no systematic study on the effectiveness of this method in the management of gastrointestinal symptoms after radiofrequency ablation.The purpose of this study is to show that traditional Chinese medicine can effectively reduce the symptoms of gastrointestinal tract during the period of atrial fibrillation ablation.AimThrough the investigation of gastrointestinal symptoms after radiofrequency ablation in patient with atrial fibrillation,to explore whether buried seeds of auricular point can effectively alleviate gastrointestinal symptoms after radiofrequency ablation in patient with atrial fibrillation,to see if the buried seeds can reduce the uncomfortable symptoms,and the influence on the psychological status of the patients and the satisfaction of treatment,try to judge the feasibility of the nursing,improve the nursing effect.Method1.Patient enrollment:all subjects for radiofrequency ablation in patients with atrial fibrillation,from Department of Cardiology,the First Affiliated Hospital of Nanjing Medical University from November 2014 to April 2015.60 patients with gastrointestinal symptoms selected,and then they were divided into a control group and the intervention group of 30 cases each.The control group used the diet nursing,psychological nursing,medication nursing,nutritional support nursing,vomiting nursing,relaxation therapy nursing.The intervention group applied the buried seeds of auricular point plus the routine nursing.2.The evaluation of Chinese version of nausea and vomiting,and retching symptoms scale(INVR)was used.Each of the cases of eligible patients was assessed with INVR scale on 19:00 every day on the first postoperative day,the second day,the third day after operation.48 hours post operation,using the Zung self-rating anxiety to assess psychological status of two groups with the same day,hospital satisfaction survey was finished on the day of discharge.The collected data were processed by using statistical software of SPSS 19,and the numeric data were analyzed by t test and the counting data analyzed by the chi square test,and the difference was statistically significant if P<0.05.ResultAll 30 cases of buried seeds of auricular point were studied.No obvious abnormal physiological measurements were found,and no serious adverse reactions were observed.The nausea and vomiting,retching scores were compared between the intervention group and control group.The first day after operation,the three dimensions of the nausea symptoms showed no differences statistically(P>0.05),however,the three dimensions of the vomiting and retching treatment group scores were lower than the control group(P<0.01),the difference was statistically significant.The data of the second day and the three day after operation showed that the scores of each dimension in the intervention group were lower than those in the control group,and the difference was statistically significant(P<0.41).The results of this study showed that the pressing of buried seeds of auricular point had better intervention effect than that of the control group,and the gastrointestinal symptoms were significantly relieved and improved after radiofrequency catheter ablation of atrial fibrillation.The anxiety scores of the intervention group were lower than those of the control group(?2=11.998,P<0.05).The total satisfaction rate was significantly higher in the intervention group(100%)than that of the control group(76.7%,?2=12.464,P<0.05).No obvious abnormal physiological parameters were observed in the patients treated with buried seeds of auricular point,and no serious adverse reactions were observed either in this intervention.ConclusionThe buried seeds of auricular point has obvious relieve effect on gastrointestinal symptoms in patient with atrial fibrillation after radiofrequency ablation,and it can reduce the patient's psychological anxiety,improve patient satisfaction rate,shorten hospitalization time,reduce hospitalization costs.In addition,it seems has no obvious adverse reaction,it is a simple and easy using method and is worth of promoting in nursing clinical practice.
