A Study On The Choice Of Medical Treatment For Five Chronic Diseases In A Grade Three Medical Institution In Jilin Province

Objective:Investigate the medical service of five kinds of disease treatment in patients with chronic diseases through the questionnaire,including the first confirmed case,medical treatment and the choice of medical institutions,the times of medical expenses,the treatment condition in half a year.In order to understand the concept of treatment in patients with chronic diseases and behavior habits,collecting public opinion for the implementation of the grading system of diagnosis and treatment,and put forward reasonable suggestions.Methods:Using the literature research method to summarize the development status of chronic diseases in China,and the summary of the chronic diseases is involved in this study.Through the way of questionnaire,investigating the medical conditions of five patients with chronic from outpatient and hospitalization in the two groups respectively,including the first confirmed case,medical treatment and the choice of medical institutions,the times of medical expenses,the treatment condition in half a year,etc.Result:1.The survey included 298 patients,according to the kinds of disease analysis,the number of the five kinds of chronic diseases were high blood pressure--96,chronic patients of diabetes--95,intervertebral disc disease--63,cerebrovascular disease--44,gastroenteritis--70.2.The first confirmed cases of five diseases:Among the 298 patients,214(71.81%),74(24.83%)and 10(3.36%)were among the patients diagnosed at tertiary hospitals,secondary hospitals and first-level hospitals.After the diagnosis,157 patients(52.68%)chose outpatient treatment(52.68%),101 patients(33.82)were hospitalized,9.06% of patients gave up treatment,and only 4.36 percent of patients were transferred to higher hospital to continue treatment.3.Cases of five chronic diseases in the last half year:Of the patients with hypertension and diabetes,72(75%)and 74(77.89%)were able to take blood pressure medicine every day according to the doctor’s advice.90 people(93.75%)and 91(95.79%)were measured in a week.The number who knows he has normal blood pressure was 60(62.5%)and 44(46.32%).There were still 20.83 percent and 10.53 percent of patients who took antihypertensive and hypoglycis only occasionally or when necessary.The rate of hypertension and diabetes prevention and control was 65.63% and 46.32% respectively.4.Treatment of five chronic diseases:Before the treatment,the patients who had not been treated,treated 1 time and treated 2 times were 56.71%,32.55% and 10.74% respectively.Among them,the main reason for not seeing the doctor is that the patient thinks the symptoms are mild,unnecessary for treatment,which accounts for 70.55%.Patients who had been treated one time had the basic choice of three tertiary hospitals and secondary hospitals,respectively 50(51.55%)and 46(47.42%).There were 43 patients(44.33%)tend to ambulatory treatment.Treatment costs can be controlled up to 500 is 32.99 percent,but there are still 27.84 percent of patients cost more than 5,000 yuan.50 percent patients who have been treated twice have seen changes at the hospital level,93.75 percent of patients choose higher-level hospitals,mainly for the purpose of seeking better therapeutic effects.5.Satisfaction of five patients with chronic diseases:298 patients’ opinion for the treatment effect,environment of treatment,service attitude,treatment cost and reimbursement rate are: satisfaction,satisfaction,satisfaction,general satisfaction and general satisfaction.Conclusion:1.It has been found that even in chronic diseases,patients will ignore primary medical institutions and are more likely to be diagnosed and treated by tertiary medical institutions.For patients has chosen the three levels of medical institutions,health policy regulation of the economic leverage effect is low,the patients think referral formalities trival,they’d like to give up the high proportion of reimbursement,and see a doctor to tertiary medical institutions directly,in pursuit of superb medical technology,well-known experts and advanced equipment and good medical environment.2.Compared to the rural residents,urban residents living standards and health awareness is higher,so the urban residents have a good health habits,are capable of measuring the blood pressure,blood sugar medicine according to doctor’s advice,on a regular basis,and have access to a chronic disease prevention and control and guidance.3.The tertiary medical institutions of this survey are representative of the Sanjia general hospital in the region,which can represent the medical level of the province.It was found in the survey that the hospital was the first to treat the patients with chronic complications,intervertebral disc disease and the acute stage of gastroenteritis.However,there is still the presence of blood glucose adjustment in the hospital,so the functional orientation of tertiary medical institutions is still the key principle of the hospital.4)The clinic frequency of patients with chronic diseases is not high,but if change hospital treatment,mainly to change a higher level of hospital,the reason for analyzing this is to the pursuit of better treatment effect,environment and equipment.5.After treatment to tertiary medical institutions,patients can get satisfactory curative effect,good medical environment and medical staff good service attitude,but they are generally satisfied with the treatment costs and reimbursement ratio,namely the high level of technology is inversely proportional with the cost of treatment.
