Investigation And Research On Medical Indigency In Tertiary Hospitals Of Fujian Province

Objective: The current status of medical indigency and the cognition of medical staff working in emergency department of these three hospitals on it are investigated and analyzed through questionnaire survey made for medical staff in emergency department of three tertiary hospitals of Fujian Province,statistic analysis of medical indigency data.The discussion of the problems existing in such various aspects as government,medical aid organizations,hospitals and patients is made in the process of solving medical indigency to put forward targeted feasible suggest and provide policy reference for relief of medical indigency,further improvement of the system in Fujian provincial medical indigency and medical assistance.Method: With combination of many research methods such as questionnaire survey,on the one hand,the data of medical indigency of three tertiary hospitals of Fujian Province from 2010 to 2017 is collected in this research,on the other hand,the cognitive situation of 254 medical staff in emergency department of tertiary hospitals on medical indigency is sampled for survey.The result of questionnaire survey is analyzed through statistical description and statistical inference.Results:(1)1316 person-times of medical indigency in three tertiary hospitals occurred from 2010 to 2017,owing money of RMB 7.435 million Yuan in total.(2)The type of medical insurance for patients with medical indigency in three hospitals is self-paid patients without exception.The main reasons for medical indigency are financial insolvency,malice arrearage,medical dispute,liability accident,patients without identity,responsible institution or personnel and payment capacity and so on.That mainly happens in ICU and surgery department.Its severity is correlated with hospital scale.(3)93.6% of medical staff in emergency department has met the patients with medical indigency.The difference among contact rate of medical staff in emergency department meeting the patients with medical indigency between different hospitals,different professions and the cognition of patients' prognosis with medical indigency,and the cognition of different professions whether the patients with medical indigency get continuous treatment in future is significant statistically.(4)75.7% of medical staff in emergency department has met the patients without identity,responsible institution or personnel and payment capacity.The difference between contact rate of medical staff in emergency department meeting the patients without identity,responsible institution or personnel and payment capacity in different hospitals and nursing methods of different hospitals for the patients without identity,responsible institution or personnel and payment capacity is significant statistically.Conclusion: The research shows that medical indigency is a phenomenon arisen out of many factors.The elimination of this kind of phenomenon needs overall planning of the departments concerned and serious implementation.In this way,it can continue to be reduced.The pressure of hospitals can be alleviated and rights of life and health for vulnerable groups in the society can be fully guaranteed.
