Objective:Through the comparision of berberine and metformin in the treatment of biochemical indicator and clinical symptom in schizophrenia with metabolic syndrome,to explore the efficacy and safety of berberine,this study is looking forward to providing a new treatment program and theoretical basis for the clinical.Methods:87 cases of patients with schizophrenia and MS were recruited in the study.Each patient was only allowed to take olanzapine.All the subjects were randomly divided into two groups.Both groups maintained on the basis of the olanzapine treatment for 12 weeks,the study group added berberine(300mg TID),the control group added metformin(250mg TID).The study group was missing 3 cases,the control group was missing 2 cases.We examined the height,body weight,BMI,waist circumference,hipcircumference,WHR,blood pressure,and scales before and after treatment for 12 th week.Taked blood samples of all patients for determination of FPG,2h PG,Hb AIC,Fins,TC,TG,HDL-C,LDL-C,Lp(a),ApoA1,ApoB,APN,LEP,Hcy,IL-1?,IL-6 and TNF-?.To assesse the treatment effect with PANSS and the clinical efficacy with CGI.To estimate adverse reactions with TESS.All the database was analyzed by statistical analysis software SPSS17.0,while P <0.05 was regarded as statistically significant difference.Result:1?Compared with before treatment,both groups had significantly decreased in height,body weight,BMI,waist circumference,hipcircumference,WHR,and blood pressure in 12 th week(P<0.05).At the end of 12 th week,there were no significant difference between two groups(P>0.05).2?In indexes of glycometabolism,the differences were remarkable of the decline of FPG,2hPG,Hb AIC,Fins,HOMA-IR levels after the treatment of two groups in 12 th week compared with the baseline(P<0.05).FPG,2hPG,HbAIC,Fins,HOMA-IR levels were no significant different between two groups after treatment(P>0.05).3?In indexes of lipid metabolism,TC,TG,HDL-C,LDL-C,Lp(a),Apo A1,Apo B levels of the two groups significantly incressed(P<0.05).All the indices between two groups had no significant difference post-treatment(P>0.05).4?Comparing with the baseline,APN,LEP and Hcy levels of two groups significantly improved(P<0.05).No significantly improvement was observed on the indices in 12 th week(P>0.05).5?The levels of IL-1?,IL-6 and TNF-? were significantly lower after treatment in both of two groups compared with the baseline(P<0.05).Compared with the control group,there was no significant difference in IL-1??IL-6?TNF-? in the study group after treatment(P>0.05).6?Compared with that before treatment,CGI score and all the item scores of PANSS of both groups have been significantly improved(P<0.05).At the end point,the PANSS positive subscale scores,negative subscale scores and the total scores of subjects receiving berberine showed greater improvement than those receiving the metformin(P<0.05).7?The TESS score had no significant difference in two groups after 12-weeks treatment(P>0.05).In the control group,the incidence rate of diarrhea decreasing was much higher than the study group(P<0.05).The other adverse reactions occurence has no statistically defference in two groups(P>0.05).Conclusion:This study prove that berberine has significant efficacy on weight loss,blood glucose and lipids metabolic abnormalities in patients with schizophrenia and MS.Berberine can improve the levels of adiponectin,leptin,homocysteine,and significantly reduce the levels of IL-1?,IL-6,TNF-?.Berberine and metformin can use to treat schizophrenia with metabolic syndrome with the same efficacy.Berberine has an obvious improvement on positive syndroms and negative syndromes,and little influence on general symptoms.Berberine combined with olanzapine is safety and has little adverse reaction.
