Objective: To analyze the relationship between the pathogenesis of recurrent aphthous ulcer(RAU)and the anxiety. And compare RAU group with control group about the salivary cortisol levels, the salivary dehydroepiandrosterone(DHEA)levels and cortisol/DHEA ratio. To further assess the correlation among scores of anxiety and the levels of salivary cortisol and DHEA and cortisol/DHEA ratio in RAU patients, and to select the important indicators which can evaluate the relation of anxiety and RAU clinically. Methods: Thirty patients with light RAU who had visited in Department of periodontal membrane of Stomatological Hospital Affiliated to Jiamusi University were selected as RAU group.Another thirty healthy persons who had clinic in The Affiliated Stomatological Hospital of Jiamusi University physical examination branch detection showed their oral mucosa healthy were recruited as control group.Collecting the saliva samples of every experment object between 8.00 am and 11.00 am.Anxiety levels of both groups were respectively evaluated by the instrument Hospital Anxiety and Depression Scale for Anxiety(HAD-A).The saliva samples were placed in-80 ? refrigerator, taking out the samples and melting them at room temperature before the experiment, centrifugal removed sediment. The method of ELISA was used to detect the levels of salivary cortisol and DHEA and to count their ratio. The statistical methods compare the differences of the anxiety scores, the salivary cortisol and dehydroepiandrosterone levels and cortisol/DHEA ratio in two groups, analyzing the correlation among degrees of anxiety and the salivary levels of cortisol, levels of dehydroepiandrosterone and cortisol/DHEA ratio in patients with RAU.Results: 1 Statistical analysis showed that the scores of anxiety was 10.53±3.73 in RAU group, the scores of anxiety was 5.47±3.56 in control group, the difference was statistically significant(P<0.05).The level of salivary cortisol in RAU group was 39.58±8.76 ug/L,which significantly higher than the controls,19.84±6.94 ug/L,the difference was statistically significant(P<0.05).The salivary DHEA level on RAU was 38.08±11.96 nmol/L,although it was lower than the controls,49.36±12.056 nmol/L,the difference was not statistically significant(P>0.05).The level of cortisol/DHEA ratio of RAU group was 1.20±0.64,which higher than the controls,0.43±0.21,the difference was statistically significant(P<0.05).2 Pearson correlation analysis revealed that the anxiety scores in patients with RAU was highly correlated with the level of salivary cortisol, and was the positive correlation, the correlation coefficient r=0.685,P<0.01.The scores of anxiety and cortisol/DHEA ratio in RAU group was highly correlation, and was the positive correlation, the correlation coefficient r=0.641,P<0.01.Conclusions: 1 The occurrence of RAU is related to anxiety.2 The level of salivary cortisol in RAU patients is increased relatively, the level of salivary DHEA showed no significant changes,cortisol/DHEA ratio is also increased relatively.3 The anxiety degrees in RAU group are correlated with the salivary cortisol levels and cortisol/DHEA ratio, the content of salivary cortisol and the cortisol/DHEA ratio can be used to evaluate the relationship between RAU and anxiety clinically.
