Using The Grounded Theory To Explore The Causes And Process Of 9 Nursing Directors' Hard Experience In Shanxi Province

Objective:A questionnaire survey was conducted to investigate whether there was a difficult experience in the work of the two level hospital and the nursing department of Shanxi province.Using grounded theory,the author explores the concrete process and causes of the hardships experienced by the director of nursing department at grade two hospital and grade three hospital in Shanxi province.In addition,from the perspective of the director of nursing,the study explored the reasons for the nursing director's love of nursing and the measures to solve the difficult work experience,which provided reference for the director of nursing department with similar experiences.Method:This study is divided into two parts:Firstly,through the convenience sampling method,whether there is a difficult experience of the director of nursing department in Shanxi Province.The results provide direction and ideas for further interviews.Secondly,using the method of Purposive sampling,Theoretical sampling and Snowball sampling.Semi structured interviews were conducted with 9 nursing directors in Shanxi Province.The interviews were recorded and transcribed,and grounded theory was used to analyze the process and the causes of the experience of the nursing director.Results:From July 7,2016 to August 7,2016,the researchers conducted a questionnaire survey of 102 nursing department directors in the Shanxi Province by WeChat platform,using a convenient sampling method,and a valid questionnaire of 75 was recovered,and the effective recovery rate was 73.53%.The results showed that: 93.3% of the nursing director of the survey,said that during the tenure of the existence of a hard experience,the average degree of difficulty score of 7.2.The study suggests that qualitative research isneeded to further explore the causes and processes.From July 2016 to February 2017,the researcher conducted in-depth interviews with the director of nursing Department of 9 nurses in Shanxi Province.Sampling was carried out by purposive sampling,theoretical sampling and snowball sampling.Before the interview,respondents were asked to fill in the basic information table and the Eysenck Personality Questionnaire.Interview 1-3 times per person.The researchers conducted a total of 11 interviews,interview time is 705 minutes,the transcription recording text of 96 thousand word,writing memos and reflective diaries of more than 6 thousand word.According to the theory of three levels of grounded theory: open coding —— selective coding —— theoretical coding,the conclusion of the experience of the director of nursing experience:(1)The process of hard experience of nursing director:Difficult events —— Cognition and interpretation —— hard experience.The results of the study also show that difficult events include: coordination;management problems;the plight of development;reform;inheritance;fight for rights.And the hard experience include: tying and bearing;no rest;fire fighters and front line commanders;resistance and helplessness.(2)The reasons for the hard experience of nursing director: Personal level: personality factors;female factors;life process factors.Hospital level: the resistance of the higher level;the communication and coordination of the parallel departments;Vulnerable group in hospital;the role of the nursing director;the complexity of the management object.Social level: nursing professional image;perceptions of nursing;Influence of nursing director.(3)The reason of persistence and love of nursing career under the hard experience:Love for nursing;responsibility and dedication;example and inheritance.(4)Measures to deal with difficult events: Get support;self evaluation and correction,work philosophy;interpersonal coordination and public relations;with the help of other forces.Conclusion:The methodology of this study is Glaser's traditional grounded theory methodology.Through interviews with the directors of 9 nursing departments in Shanxi Province,we come to the conclusion that there are six kinds of hardships and four difficult experiences in the work of the nursing director.Three levels of coding extracted two key concepts of "hard perception" and "difficult environment",and drew the model map of hardship experience,which deepened the understanding of this problem and brought about thinking and change.
