Objectives:To explore the understanding of nursing difficulty of medical staff, and to build a theoretical framework about the nursing difficulty.Methods:This study used qualitative research approach. Applying the principles and procedures of grounded theory, This study selected 40 medical staff of tertiary referral hospital through purposive sampling, theoretical sampling method, and the reseacher conducted semi-structured in-depth interview on every interviewees, using "the understanding and factors that influenced nursing difficulty " as the theme of the study subject. All recorded data were transcribed and saved by word document, then the collected data was analysed and sorted strictly in accordance with grounded theory coding strategy.Results:This study summarized by the "patients' situation", "nurses' diathesis", "organizational Factors" and “materials and equipment factors” as four main factors.The "patients' situation" included patients' condition, age, self-care ability, compliance,social support system, psychological condition, requirements and expectations. The "nurses' diathesis" consisted of nurses' work experience, knowlegde and skill, as well as communication skills. The resoures and workload of nursing, the Coordination of doctors and ancillary departments are the elements of the "organizational Factors".Conclusion:This study defined the concept of nursing difficulty based on the factors for the first time, nursing difficulty refers to the complexity of nursing tasks during the implementation of care plans to the patients, which influenced by the patients' situation,nurses' diathsis and other factors from many aspects and multidisciplinary. The study also builded the theoretical framework for nursing difficulty, it consists of the factors above and the consequence of the increased difficulty of care, which includedpatients' increased demand of care, increased workload and nursing human resourcesas well as the increased consumption of medical resources.The theoretical framework for nursing difficulty provide a theoretical basis and foundation for the further study and future nursing research of the evaluation of nursing difficulty.
