Objective:In this study,we designed a surgical instrument—residual-shown device,which is mainly used in pediatric surgery for exposing the root of the processus vaginalis stump directly and apply it to the clinic,comparing with other instruments like vascular clamps and tractors.Checking out whether it is worthy of clinical promotion.Methods:1.Design a surgical instrument sample figure,and the instrument has three different sizes of models.Taken stainless steel or aluminum alloy as material.The equipment is sterilized by process and applied to expose the root of the processus vaginalis stump during the operation.As shown in the figure below,the device consists of three parts:A-side,the connecting part,B-side.The key is A and B-end.A-side is a flat ellipsoid with a smaller groove;the groove provides operating space for the gradual transition of the outer large;on the both flanks of A-side are barriers;connector is a rectangle with small head and large tail connecting A and B-end;B-end's structure is as same as A-end,taking a larger groove flat ellipsoid;the groove provides bigger operation space;on the both flanks of B-side are larger barriers.2.93 boys suffering from unilateral hernia(63cases)or unilateral hydrocele(30cases)were chosen from January 2016 to April 2017 in the 1st hospital of Shanxi Medical University.Divided into hernia group and hydrocele group.Experimental group and control group were made randomly according to the difference of the using of instruments for exposing processus vaginalis stump.Analyzing the effects of the using of residual-shown device by the operator subjectively and recording operation time.Results:1.The residual-shown device designed by the subject has a better subjective evaluation of its effect on exposing the root of the processus vaginalis stump(P <0.05).2.Patients with unilateral oblique hernia group:the operative time which using of the residual-shown device to expose the root of the processus vaginalis stump(15.41±3.89)min shorter than that of vascular clamps and tractor to expose the stump(19.68±5.55)min(P < 0.05);Patients with unilateral hydrocele group:the operative time using the residual-shown device to expose the root of the processus vaginalis stump(17.19±4.46)min shorter than that using vascular clamps and tractor expose the stump(21.00±5.63)min(p < 0.05).3.Patients with unilateral oblique hernia and hydrocele group:the operative time is basically the same as which using of the residual show device to expose the root of the processus vaginalis stump.The difference is not statistically significant(P?0.05).Patients with unilateral oblique hernia and hydrocele group:the operative time is basically the same as which using of vascular clamps and tractor to expose the stump.The difference is not statistically significant(P?0.05).Conclusion:The device-residual-shown device is easily to use and it can better help operator to exposure the root of the processus vaginalis stump,and is worthy of clinical spreading.
