Objective In order to explore the surgical treatment in vaginal stump recurrent carcinoma in the role and influence on prognosis of tumor.Statistical analysis of cervical cancer after primary surgical treatment of VSRC in patients with clinical and pathological data characteristics,the comparison between surgical treatment method and the traditional radiotherapy, chemotherapy,influence of the related factors of surgical treatment on prognosis to the outcomes. To provide the reference basis for the treatment of local recurrence of vaginal redidue of cervical cancer for the patients.Methods Twenty-two cases with cervical cancer(CC) after radical hysterectomy and pelvic lymphadenectomy admitted in our hospital from 2002 to 2014 were retrospectively analyzed. Divided the patients into different treatment groups,the corresponding complications caused by different treatments groups,different clinical stages for the first time groups,different initial pathology classification groups,with or without high risk factors,groups accepted radical radiotherapy or not after first radical hysterectomy.Obtain the survival rates and time analysis results between each groups.Results The 2-year-survival rates of the different groups of surgical treatment and radiotherapy, chemotherapy respectively are 71.3% and 65.8%,the comparison of 2-year-survival rates between two different treatment methods has significant difference(p < 0.05). The 2-year-survival rates of the different groups of intraoperative hemorrhage and injury and complications occurred secondary to radiotherapy respectively are 69.5% and 68.4%,the comparison of 2-year-survival rates between two different treatment complications has no significant difference(p > 0.05), further more,the incidence of the different groups of intraoperative hemorrhage and injury and complications has no significant difference(p>0.05). The incidence of the different groups of post-operative complications and long-term complications about post-radiotherapy and chemotherapy respectively are 4.5%、4.7%. It has no significant difference(p>0.05).The amount of different clinical stages for the first time respectively are 10 cases ofⅠB1 period(45.5%),6 cases ofⅠB2 period(27.3%),4 cases of ⅡA period(18.1%),2 cases of ⅡB period(9.1%), 2-year-survival rates respectively are 90.4%、84.8%、74.7%、68.6%,average survival time respectively are(39.5±3.3)months、(34.3±1.4)months、(24.2±1.3)months、(20.1±0.3)months,the comparisons of survival time and rates have significant difference(p<0.05).The amount of primary tumor tissue pathologic differentiation degrees are 15 cases of G1 level(68.2%),3 cases of G2 level(13.6%),4 cases of G2 level(18.2%), 2-year-survival rates respectively are 90.6%、84.5%、55.7%,average survival time respectively are(38.3±2.3)months、(34.1±2.5)months、(17.2±2.6)months,it has significant difference(p<0.05).There are 8 cases(36.4%)with high-risk factors,1 case(4.5%)with vessel invasive status,3 cases(13.6%)with tumor size greater than 40 mm,2 cases(9.1%)with lymphatic vessel invasion,2 cases(9.1%)with cervical mesenchyme deep muscle infiltration,and 14 cases( 63.6%) without high-risk factors.The 2-year-survival rates between two different groups respectively are 59.5%、89.5%,average survival time respectively are(24.5±3.8)months、(36.5±4.4)months,the comparisons of survival time and rates have significant difference(p<0.05).7 cases(31.8%)accepted radical radiotherapy and 15 cases( 68.2%) does not accept radical radiotherapy,2-year-survival rates respectively are 85.5% 、 67.5%,average survival time respectively are(39.5±3.4)months、(39.5±3.4)months,the comparisons of survival time and rates have significant difference(p<0.05).Conclusions The 2-year-survival rates of cervical cancer after primary surgical treatment of VSRC in patients preferably have surgical treatment are significantly higher than patients with radiotherapy and chemotherapy in literature reports.The comparisons between intraoperative hemorrhage and injury and long-term complications of surgical treatment in vaginal stump recurrent carcinoma and complications of radiotherapy 、chemotherapy have no significant difference.The effect of surgical treatment on the postoperative prognosis of 22 cases of vaginal stump recurrent cancer are ralated to clinical stages for the first time 、high-risk factors、primary tumor tissue pathologic differentiation degrees、accepting radical radiotherapy status after primary surgical treatment.
