Analysis Of The Factors Related To The Prognosis Of Early Microsurgery For Ruptured Anterior Circulation Aneurysms

Objective:Analyze the effect of early microsurgery for ruptured anterior circulation aneurysms,and to explore the factors related to the prognosis of patients with ruptured anterior circulation aneurysms treated by early microsurgery? Methods:Clinical data of 89 patients with ruptured anterior circulation aneurysms treated by early microsurgery from 2014 to 2016 in the Department of Neurosurgery of the First Hospital of Shanxi Medical University were collected,Patients were evaluated according to GOS score,and SPSS19.0 statistical software was used to analyze the age,gender,hypertension,diabetes,preoperative Hunt-Hess grade,modified Fisher grade,preoperative rebleeding,aneurysm size,aneurysm neck width,intraoperative aneurysm rupture,temporary occlusion,delayed cerebral ischemia,pulmonary infection,intracranial infection,postoperative cerebrospinal fluid circulation disorder and delayed bleeding of 16 factors that may affect the prognosis of patients,c2 test and Logistic regression analysis were used for univariate analysis and multivariate analysis respectively? Results:Among the 89 patients with ruptured anterior circulation aneurysms treated by early microsurgery,the prognosis of 58 patients was good(65.2%),and the other was poor(34.8%).Univariate analysis showed that the age,hypertension,neck width,preoperative Hunt-Hess grade,modified Fisher grade,delayed cerebral ischemia,pulmonary infection and postoperative cerebrospinal fluid circulation disorder are associated with the prognosis of patients(P < 0.05),Logistic regression analysis showed that preoperative Hunt-Hess grade(OR=8.994,P=0.048),delayed cerebral ischemia(OR=7.586,P=0.027)and pulmonary infection(OR=8.323,P=0.004)were risk factors affecting the prognosis of patients with ruptured anterior circulation aneurysms treated by early microsurgery?Conclusion:Early microsurgery for ruptured anterior circulation aneurysms can achieve good effect,preoperative Hunt-Hess classification,delayed cerebral ischemia and pulmonary infection were risk factors affecting the prognosis of patients with ruptured anterior circulation aneurysms treated by early microsurgery?...
