Objects To screen the predictive risk factors affecting outcome of anterior circulation aneurysms in which microsurgery were performed and establish a statistical model to predict the prognosis.Methods Surgical outcomes were assessed 3 months after discharge using the Glasgow outcome score (GOS).Possible influencing factors including the age and gender of patient, hypertension, the consciousness loss at onset, intracranial hematoma before treatment, the aneurysm site, pre-operative Hunt and Hess grading, the timing of surgery, intraoperative rupture and the largest diameter of aneurysm were analyzed with binary logisitic regression analysis model in SPSS16.0.Results A total of 119 patients with single rupture anterior circulation aneurysm received microsurgery. Of those,89 patients (74.8%) had a favourable outcome (GOS 4-5), while 30 patients(25.2%) had a poor outcome(GOS 1-3). Logisitic regression analysis shows independent variables affect outcome including age(P=0.024), intracranial hematoma before treatment(P=0.001), pre-operative Hunt and Hess grading (P=0.000) and intraoperative rupture(P=0.030).Conclusion age> 65, complicating with intracranial hematoma, high pre-operative Hunt and Hess grading and intraoperative rupture were associated with an increased risk for poor outcome after microsurgery of anterior circulation aneurysms. We can predict the prognosis by assessing these risk factors.
