ObjectiveTo synthesis and analyze various factors influencing the postoperative prognosis of anterior circulation aneurysms clipping, we hope to found some statistically significant indicators so as to guide the surgerical therapy and ultimately help the patients to gain the best curative effect of the operation. MethodsFrom 2012 January 01, 2013 to December 31, Clinical data of 98 patients with intracranial aneurysms of anterior circulation form Department of Neurosurgery, the First Affiliated Hospital of Zhengzhou University has been Retrospective Summarized.Follow-up for patients who were discharged from hospital after 6- 12 months, according to the patient’s gender distribution, age differences, whether it is history of hypertension, smoking history, with Fisher and hunt Hess grade, patients of aneurysm number, specific location, specific size and surgical factors(the timing of operation, intraoperative aneurysm rupture and intraoperative parent artery occlusion)were divided into two groups, and compared for each group, the Glasgow Outcome Scale(Glasgow Outcome Scale(GOS) score results. The effect of various factors on the clinical outcome of clinical prognosis was verified by statistical method. ResultsSingle factor analysis results show that: ①There were significant differences in the GOS scores of patients with different Hunt-Hess grades(P<0.05).;②There were significant differences in GOS scores between different patients, and were statistically significant(P<0.05);③There were no significant differences in the GOS scores of patients with hypertension history, and no significant difference(P>0.05);④Results the GOS score of patients without smoking history were not significant, and no statistical significance(P>0.05);⑤The number, location and the maximum tumor diameter of the capsule in patients with different GOS scores were no significant difference, and there is no statistical significance(P>0.05);⑥Results the GOS scores of the different timing of surgery in patients with no significant difference, and there is no statistical significance(P>0.05);⑦The results of GOS surgery without rupture of aneurysm patients had no significant difference, and there is no statistical significance(P>0.05);⑧With the GOS score and intraoperative underwent temporary occlusion of the difference is not significant, and no statistical significance(P>0.05); ⑨ There was no significant difference in patients with different gender in prognosis(P>0.05).⑩The relationship between different Fisher grade and curative outcome after surgery(GOS) analysed by means of the rank sum test for two samples. The results that P<0.01 means statistically significant difference was existed. The outcome shows that patients with pure subarachnoid hemorrhage(SAH) confirmed by preoperative cranial CT have better prognosis than patients with intraventricular hemorrhage and / or intracerebral hematoma. The analysis results show:Hunt-Hess grade and Fisher grade are independent risk factors influencing the short-term outcome of aneurysmal patient. ConclusionHunt-Hess grade and Fisher grade are independent risk factors influencing the curative outcome after aneurysms clipping. And patients with temporary interruption of gender, age, history of hypertension, smoking history, aneurysm quantity, location of aneurysm, arterial aneurysm size, operation time, operation and intraoperative aneurysm rupture are not influence risk factors for the prognosis of patients.
