The Applied Study Of Caries Risk Assessment And Management In Primary School Oral Public Health Project

Objective: To optimize the use of limited oral hygiene resources,this subject will explore the application of dental caries risk assessment and management in primary school oral public health programs and provide reference for future oral public health programs.Methods: The risk assessment and management standard of dental caries used in oral hygiene were established by searching literature,consulting experts and combining the epidemic characteristics of children's caries in our city.A total of 3591 students from 1 to 5grade in 7 primary schools in Huichuan District of Zunyi City were collected by stratified cluster sampling method in Huichuan City,Zunyi City.According to the assessment of dental caries risk in this study,children were examined for oral examination and caries risk factors to assess the risk of dental caries in children: low risk,moderate risk or high risk.The children are managed by the risk assessment level:High risk children are given oral health education,local fluorosis,PFS and ART intervention;moderate risk children were given oral health education,local fluoride and ART intervention;low risk children were randomly divided into group A and group B.Group A received oral health education;Group B was given oral health education,local fluoride and PFS.The assessment of dental caries risk assessment and management was conducted by assessing the prevalence of oral health knowledge before and after oral health education,the incidence of dental caries and the cost of dental prevention interventions after one year to assess the effect of the application of public health projects.Results:1 This study studies 3591 students,male 1886 people,women 1705,in 7 primary schools in Zunyi City Huichuan District.2343 children suffer from caries.The caries rate is65.25% and caries were 2.79.The filling composition ratio of dental caries is 4.07%.2 In the total of 3591 subjects,there are 906 people in low risk,1281 people in risk,and1404 people in high risk.The number of dental caries in low,medium and high risk caries was 0,1876 and 8150 respectively,and the caries are 0,1.46 and 5.80 respectively.Therewas no significant difference between males and females(P> 0.05).There were differences in caries risk according to the different grades and if they are the children of migrant workers(P <0.05).Students from the 1st to the3 rd grade and from the migrant families are is high risk of caries.3 The incidence of caries in children with low risk group A,low risk group B,middle risk group and high risk group was 5.99%,5.57%,6.59% and 8.03%,and there was no statistical significance(P> 0.05).The new caries are 0.08,0.08,0.09 and 0.12.There was no significant difference(P> 0.05).4 The master of oral health knowledge in group A,group B,middle risk group and high risk group was 72.22%,72.08%,56.62% and 47.44%,and there was significant difference between groups(P <0.05).The correct brushing rate was 75.35%,75.13%,47.96% and36.13% and there was difference between groups(P <0.05).The master of oral health knowledge in group A,low risk group B,middle risk group and high risk group was88.40%,89.20%,87.66% and 87.78% after oral health education,the difference was not statistically significant(P> 0.05).The correct brushing rate was 81.67%,82.41%,81.74%and 83.02% and the difference was not statistically significant(P> 0.05).5 According to the analysis of the cost of this study to oral examination and risk assessment,oral health education,local fluoride,pit and fissure closure and ART,the average cost was 5.26 yuan,1.44 yuan,32.71 yuan,35.63 yuan and 46.40 yuan per person.The average cost of low risk group A,low risk group B,middle risk group and high risk group was 6.70 yuan / person,76.18 yuan / person,57.30 yuan / person,115.40 yuan /person.Conclusion:1 According to the dental caries risk assessment,high risk of caries mainly concentrated in the first grade to third grade and children of migrant families.We suggest that this part of the children should be focused on prevention and treatment.2 The application of the risk assessment and management of dental caries in the school oral public health projects has a reasonable role in the distribution of oral medical resources and it makes the children in high risk of caries be in key prevention and control,so as to control the occurrence and development of dental caries in children.It is worthgeneralizing in the oral public health project.
