Objective To investigate the caries status and caries activity of 3-year-old children in Hohhot area,and to follow-up for 9 months,to observe the occurrence and development of caries in children and the changes of caries activity determined by Cariostat test.The relationship between caries status and caries activity was evaluated.The accuracy and effectiveness of Cariostat test to reflect the status of caries and predict the risk of caries were evaluated.Methods Randomly selected 256 3-year-old children from multiple kindergartens in Hohhot,and performed 1.Oral examination: Fill in the oral examination record form,and calculate the number of dental caries(dft)and dental caries severity index(CSI).2.Cariostat caries activity test: Cariostat reagent was used to test the children.Caries status value(CAT)? 1.0 was recorded as a low risk of caries;caries status value ? 1.5 was recorded as a high risk of caries.3.Parents of children to be tested should complete the caries susceptibility risk assessment scale.4.According to the results of oral examination and Cariostat test,the caries-free children were divided into two groups with low risk of caries and high risk of caries.After 9 months,the incidence of new caries in different caries status groups was observed and compared;Children were examined for caries status and caries activity again to observe the occurrence and development of caries and the changes of caries status.5.Statistical analysis: The experimental data was analyzed by SPSS 20.0 statistical software,the count data was expressed by composition ratio,and the difference analysis between groups was by chi-square test.If the measurement data obey the normal distribution,it is expressed as mean ± standard deviation,the difference between the two groups is tested by independent sample t test,and the comparison of three groups or more is one-way analysis of variance;if it is not normal,the quartile is used.The number indicates that the non-parametric test was used to analyze the differences between the groups.To determine whether the diagnostic method has diagnostic value,the ROC curve and related indicators are used for evaluation.Binary logistic regression analysis was used to analyze the independent influencing factors of caries occurrence.The difference was considered statistically significant at P <0.05.Results 1.Caries status and Cariostat test: 256 children were investigated in this study.The initial caries rate was 74.6%,dft3.79 ± 3.81,CSI10.0 ± 10.0,CAT1.09 ± 0.79;There were no significant differences in dft,CSI,and CAT values between different genders;there were statistically significant differences between different hukou types.2.The distribution of dft and CSI in CAT: The distribution of dft and CSI in CAT is statistically significant.There is a positive correlation between CAT,dft,and CSI indicators.3.The accuracy of the Cariostat test as a caries screening tool: the sensitivity is 59.5%,the specificity is 94.4%,the false negative rate is 27.6%,and the false positive rate is 98.5%.4.Relevant factors affecting children's caries:(1)The results of single factor analysis show whether parents like to eat sweet snacks or drink sugary drinks frequently,family economic status,whether parents have received worm tooth treatment,parents brush their teeth daily,and whether children have Drinking milk at night,children 's diet,whether parents brush their children 's teeth,and whether children have been treated with fluoride at specialized hospitals affects children 's caries.(2)The results of Logistic regression analysis showed that the main risk factors affecting children's dental caries were family economic status,the frequency of daily toothbrushing by parents,children's drinking habits at night,and fluoride application experience.Follow-up after 5.9 months: The incidence of new caries in non-caries children in the high-risk group after 9 months was lower in the lower-risk group,and the difference was statistically significant;the caries rate of all children examined after 9 months was 80.9%,dft5.37 ± 4.85,CSI14.84 ± 13.16,and CAT1.48 ± 0.92.Compared with the first time,the overall caries rate,dft,CSI,and CAT values of the tested children showed an upward trend,and the differences were statistically significant.Conclusions 1.The prevalence of caries in children aged 3 years in Hohhot is higher than the national average.The status of caries and caries activity is related to the type of hukou,and has nothing to do with gender.2.The Cariostat test can reflect the current status of dental caries in children.The difference in the distribution of dft and CSI in each index of CAT is statistically significant.There is a positive correlation between CAT,dft,and CSI indicators.3.The Cariostat test has certain clinical diagnostic value as a caries screening tool.4.Do parents like to eat sweet snacks or drink sugary drinks often,family economic status,whether parents have received worm tooth treatment,parents brush their teeth daily,whether children have the habit of drinking night milk,children's diet,whether parents brush their children's teeth,children whether or not the application of fluoride to professional hospitals affects children's caries;among them,family economic conditions,the frequency of parents' daily tooth brushing,children's drinking habits of night milk,and fluoride application experience are the main risk factors that affect children's dental caries.After follow-up of 5.9 months,the incidence of new caries in children without caries in the high-risk group was more serious than in the low-risk group,confirming the effectiveness of the Cariostat test in predicting caries;After 9 months,the caries rate,dft,CSI,and CAT of all tested children showed an upward trend,and the differences were statistically significant,indicating that over time,the degree of deciduous tooth caries increased,and caries activity increased,and should be early take precautionary measures.
