Objective:Meniscus is an important component of knee joint.Meniscus injury is very common.It often causes irreversible degeneration for arthrodial cartilage,and eventually leads to osteoarthritis of knee joint.Currently there are many ways of treatment for meniscus injury.In clinical practice reparative therapy is commonly used.There are many ways for meniscus suture,including outside-in suture,inside-out suture and all-inside suture.The effectiveness of suture methods for different parts of body reported according to clinical practices differs from one to another.As for meniscus injuries with suture indications,each of the above three suture method has its own advantages and disadvantages.Outside-in suture is good for the anterior horn of the meniscus tear and easy to operate;Fast-fix is good for posterior horn tear and effective.Inside-out suture is regarded as golden standard for body tear and commonly applied.As arthroscope suture apparatus improves,Fast-Fix becomes more popularized due to its easy operation and reliable effect.The research aims to evaluate and make a contrast on clinical effects of two suture methods for lateral meniscus tear,and to offer guidance for clinical practices.Research Methods:1.We collected 40 cases of lateral meniscus tear patients with suture indications between September,2012 and September,2013.We put them to 2 groups,20 cases in each group.We evaluated them with IKDA,Lysholm and kept records.2.For Group A,Fast-Fix suture method was applied;for Group B arthroscope Inside-out suture method was applied.3.After operation we designed standard personalized recovery plan and conducted recovery training.We followed up for 36 months.We called back and rechecked them respectively the 1st,3rd,6th,12 th,18th,24 th and 36 month after operation,and we evaluated results with IKDC and Lysholm.After that,we kept a record.4.We conducted statistical analysis and contrastive study with SPSS17.0.Findings:Operation,Length of stay and Postoperative signs: On bleeding amount during operation,length of stay in hospital and postoperative signs,there is no static significance between the two groups(P> 0.05).On operation time,Group B takes less time than Group A.Therefore there is static significance.Postoperative knee joint evaluation: we followed up respectively 1 month,3months,6 months,12 months,18 months,24 month and 36 months after operation.We evaluated patients with IKDC and Lysholm.On knee joint function,both groups had higher scores than before operation.There is no static significance for difference between 2 groups 6 months,12 months,18 months,24 months and 36 months after operation.Conclusion:Inside-out suture and Fast-Fix suture have similar clinical effect for lateral meniscus tear.After surgery patients get higher score in knee joint evaluation than before surgery.It has satisfactory clinical effect.However,the latter is easy to operate,and takes less time for operation.Meantime,high cost and removable flexibility after being placed in articular cavity are its disadvantages.Inside-out suture takes longer time during surgery and its operation is complicated.On the other hand,it is easier to adjust it suture direction and suture stitches during surgery.And also knot is placed out of joint capsule and does not affect arthrodial cartilage.
