ObjectiveTo explore the main influence factors of alexithymia and resilience in cancer patients,focus on the level and relationship among alexithymia,resilience and quality of life of them;to test a hypothesis that perceived resilience mediates between alexithymia and quality of life.In order to improve the the level of resilience,alleviate its alexithymia and further improve the quality of life as well as provide valuable theoretical basis for cancer patients.Methods1.Literature analysis was conducted to explore the influencing factors of alexithymia and resilience,summarize the appropriate process of resilience and alexithymia and theoretical foundation of this study.2.A total of 376 patients with cancer in two third-level hospitals in Taian city were investigated with cross-sectional survey by using Connor-Davidson Resilience Scale(CD-RISC),Toronto Alexithymia Scale(TAS-20),Chinese Strategies Used by People to Promote Health(SUPPH),Social Support Rating Scale(SSRS)and Functional Assessment of Cancer Therapy-General(FACT-G),to test the alexithymia,resilience,selfcare self-efficacy,social support and quality of life of cancer patients.Data were processed using SPSS(Statistical Package for the Social Sciences)17.0 software.Pearson correlation techniques were used to examine the relationship between quality of life,alexithymia and resilience;Multiple stepwise regression were used to examine the influence factors of alexithymia and resilience;and the proposed hypothesis was tested by structural equation model.Results1.By literature review and theoretical analysis,an initial conceptual framework was developed to guide the whole research.2.The total score of alexithymia in cancer patients was 57.30±6.26,the average score of difficulty identifying feelings was2.92±0.55,the difficulty describing feelings was 2.65±0.41 and externally oriented thinking was 2.83±0.32,the influence factors of alexithymia were self-care self-efficacy,self-decision,objective support,positive attitude,degree of education,marriage,residence,medical payment method.All variables entering the model could explain 64.8%(Adjusted R2=0.648).The total score of resilience of cancer patients was 59.17±8.93,the average score of tenacity,strength and optimism was 2.31±0.39,2.43±0.43,2.43±1.57 respectively,the influence factors of resilience in cancer patients were self-care self-efficacy,gender,family per capita income,religion,social support,degree of education and treatment.The total score of quality of life was 59.69±13.78,the average score of each subscale were as follows,the physiological condition was 2.49±0.65,society family condition was 2.63±0.54,emotional condition was 2.23±0.65,function condition was 1.50±0.64.3.Cancer patients' quality of life was negatively correlated with the alexithymia(r=-0.695,P<0.01),was positive correlated with resilience(r=0.450,P<0.01),the level of resilience was negatively correlated with the alexithymia(r=-0.452,P<0.01).Path analysis showed that both the level of alexithymia(?=-0.556,P<0.01)and resilience(?=0.318,P<0.01)had directly effects on quality of life of cancer patients,resilience mediated the relationship between alexithymia and quality of life(?=-0.128,P<0.05),the mediation effect accounted for 18.9% of the total effects.Difficulty identifying feelings had directly negatively effects on each subscale of quality of life,includes physiological,society family,emotional and function,and through the positive indirect impact on social family,emotional and functional condition of subscales of quality of life,the effect was-0.027,-0.027 and-0.026 respectively;Difficulty describing feelings had only directly negatively effects on emotional condition,the effect was-0.279;Difficulty describing feelings also through the strength of the mediation effect on the social family,emotional and functional condition of each subscale of quality of life,the effect was-0.032,-0.028,-0.042 respectively;The subscale of externally oriented thinking had only directly negatively effects on social family and functional condition,but also indirectly through the mediation effect of strength and optimistic influence social family,emotional and functional status.Conclusions1.The level of resilience and quality of life in cancer patients' were low,and the level of alexithymia was moderate.2.The level of alexithymia was negatively correlated with the resilience and quality of life of cancer patients,the level of resilience in cancer patients was significantly positivly correlated with quality of life.3.The influence factors of alexithymia were self-care self-efficacy,self-decision,objective support,positive attitude,degree of education,marriage,residence,medical payment method;the influence factors of resilience in cancer patients were self-care self-efficacy,gender,family per capita income,religion,social support,degree of education and treatment.The same influence factors of alexithymia and resilience were self-care self-efficacy and degree of education.4.Resilience could partly mediates between alexithymia and quality of life;resilience could positively predict quality of life,alexithymia could negatively predict quality of life,and also negatively predict resilience.Difficulty identifying feelings could directly predict each dimension of quality of life,strength and optimistic also has partial mediates effect.Increase the level of resilience of cancer patients could slow the negative impact of alexithymia on the quality of life.
