Objective:The questionnaire was used to investigate the status of depression in the elderly population of Taiyuan City,analysis of influencing factors of depressive symptoms in the elderly;on this basis,set up a set of targeted and operable nursing intervention program based on the hopelessness and self-esteem theories,and to explore the intervention effect of the nursing program on depression symptoms in elderly.Methods:The study includes two stages: current situation investigation and nursing intervention.The current investigation stage: 234 elderly people from two pension institutions in Taiyuan city were selected as the research object by the convenience sampling method.Using the Geriatric Depression Scale(GDS-30),the self-made general data questionnaire,the revised Geriatric Attributional Style Questionnaire(GASQ),the Rosenberg Self-Esteem Scale(RSES),and the Life Events Scale for the Elderly(LESE).A baseline survey of older adults was conducted by trained investigators using one-to-one method,To understand the current situation of depressive symptoms,to analyze the factors that affect the depressive symptoms of the subjects(risk factors and protective factors),At this stage,the elderly depression patients were screened.Nursing intervention research stage: 104 cases of depression were screened by the agency as the intervention group and the control group,were given different interventions.Psychological nursing and health education in control group were given routine;the intervention group received nursing intervention program pre-establishedon the basis of the hopelessness and self-esteem theories,including Disease related knowledge,attribution retraining(attribution style knowledge,individual attributional style analysis,The guidance of positive attribution)self-esteem training,psychological care,dietary guidance,exercise guidance,The organization of recreational activities(talent show,collective viewing,etc.)V and other characteristics of nursing intervention.Before and after intervention,The Geriatric Depression Scale(GDS-30),Rosenberg Self-Esteem Scale(RSES),revised Geriatric Attributional Style Questionnaire(GASQ)were used to evaluate the intervention effect of each group.Results:1.A total of 228 elderly people completed the survey,124(54.4%)normal old people,104(45.6%)depressed elderly,Among them,84 were mild depression(36.8%),moderate to severe depression were 20(8.8%).2.Single factor analysis showed: age,occupation before retirement,monthly income,education level,The number of chronic diseases,marital status,living alone,sleep quality,recreational activities,physical exercise,Frequency of children's visit,The influence of childhood experience,self-esteem,attributional style,negative life events and Subjective evaluation of depression are independent factors of geriatric depression(P<0.05),However,there was no clear relationship between gender,religious beliefs,drinking or smoking and the prevalence of GD(P>0.05).3.The results of multivariate analysis showed that age(OR=0.481,95%CI=0.289-0.800),sleep quality(OR=0.519,95%CI=0.270-0.998)and degree of education(OR=0.480,95%CI=0.278-0.827),the number of chronic diseases(OR=2.227,95%CI=1.248-3.975),entertainment(OR=0.438,95%CI=0.215-0.893),frequency of children's visit(OR=0.232,95%CI=0.093-0.580),subjective evaluation of depression(OR=4.154,95%CI=1.295-13.324),The level of self-esteem(OR=0.890,95%CI=0.795-0.997),the influence of n egative events(OR=1.052,95%CI=1.003-1.102)and attribution style(OR=0.938,95%CI=0.900-0.978)were independent factors of GD(P<0.05).Among them,aging,high ed ucational level,often participate in recreational activities,children often visit,high s elf-esteem and positive attribution is a protective factor for GD;chronic disease,su bjective evaluation of melancholy,negative life events were the risk factors of GD.4.Before the intervention,no statistically significant differences between the two groups of subjects of general information,GDS-30 scores,Rosenberg Self-Esteem Scale scores and GASQ scores(P>0.05);after the intervention two groups in GDS-30 score,GASQ score and each dimension(except towards global-specific dimensions of GASQ)had significant difference(P<0.05),no difference(P>0.05)in Rosenberg Self-Esteem Scale score.After intervention,the intervention group of the elderly in the total score of GDS-30,GASQ scores and scores of each dimension,Rosenberg Self-Esteem Scale score was statistically significant(P<0.05).Conclusion:1.The elderly people in pension institutions in Taiyuan have higher levels of depression symptoms,and was higher than that of similar research.Early screening and follow-up should be carried out in a timely manner.2.Negative attributional style,low self-esteem,negative life events,etc..It is necessary to carry out preventive nursing intervention for this kind of high risk factors.3.The nursing intervention program based on the theory of hopelessness self-esteem is feasible after empirical research,it can be extended in the pension institutions.4.The nursing intervention can improve the attribution of the elderly,improve the self-esteem level of the elderly and alleviate the symptoms of depression in the elderly.
