Objectives(1)Figure out the current medicine-taking situation of the elderly patients with chronic diseases in pension institutions,and provide realistic basis for strengthening endowment institutions' intervention on the elderly patients with chronic diseases.(2)Analyze the factors that affect the medication compliance of elderly patients with chronic diseases in pension institutions,and reveal the essential group and related care services of medication compliance intervention on elder people with chronic diseases.Methods Cross-sectional study method and convenience sampling are adopted.The elderly living in the endowment institutions in Zhangye,Pingliang and Lanzhou the three cities in Gansu Province who suffer from chronic diseases such as hypertension,diabetes,cerebrovascular disease,ischemic heart disease and chronic obstructive pulmonary disease,and agree to participate in this research are taken as research objects for this study.The research tools include the general demographic data,the sickness status and the medicine-taking situation of the elderly patients with chronic diseases in the endowment institutions,as well as the Chinese version of Beliefs about Medicines Questionnaire-Specific,the Chinese version of 8-item Morisky Medication Adherence Scale,the Medication Knowledge Questionnaire,Europe health-related quality of life questionnaire 5 dimensions,etc.All the data collected is double entered by using the EpiData3.0 software and analyzed through the SPSS22.0 software.Descriptive statistical analysis,single factor analysis,variance analysis,T-Test,Pearson correlation analysis and multiple linear regression analysis are also conducted on the data.Results(1)The score for the medicine-taking compliance of the elderly patients with chronic diseases in pension institutions is(6.07 ± 1.91).There are significant differences in medicine-taking compliance among those patients with different education levels,occupations,reasons for living in endowment institutions,drinking status,exercise status,understanding level of diseases,symptoms,impact on physical and mental health,physical examination status,the degree of wrong medication,the degree of self-medication,and the efficacy of medicine(p <0.05).(2)The medication compliance and medication knowledge of elderly patients with chronic diseases in pension institution(r=0.236)are positively correlated(p<0.01).The medication compliance(r=0.216)and medication belief together with one of the dimensions: belief in the need for medication(r=0.151),are positively correlated(p<0.01).The same positive correlation relationship applies to health description system(r=0.265)and visual simulation scale(r=0.300)are positively correlated(p<0.01).(3)The factors affecting the medicine-taking compliance of the elderly patients with chronic diseases in pension institutions mainly include self-medication(?=0.218),self-assessment of health status(?=0.189),beliefs about medicines(?=0.174),understanding level of diseases(?=-0.169),reasons for living in an endowment institution(? =-0.126),education level(? = 0.122),and degree of wrong medication(? =-0.103).Conclusions The elderly patients with chronic diseases in pension institutions have a low medicine-taking compliance,and there are some unreasonable medicine-taking behaviors.Their medicine-taking compliance remains to be further improved.There are obvious differences in medicine-taking difference among different features.It is recommended that pension institutions should pay attention to the medication problem of the elderly,establish a specialized care team,and incorporate the medication service into daily services.In addition,these institutions are also advised to strengthen the health education of the elderly and improve their medicine-taking compliance.
