Objective:Established the SD rat model of type 2 diabetes mellitus with chronic periodontitis,to explore the influence of periodontitis on type 2 diabetes vascular lesions by observing vascular pathological changes,and detecting quantitative analysis periodontal bacteria in carotid artery tissue.Discuss the influence of oral interventions on glycated hemoglobin level and vascular tissue in chronic periodontitis SD rats associated with type 2 Diabetes Mellitus.Methods:SPF SD rats were randomly divided into three groups,group A(the normal control group,NC)and group B(diabetes mellitus group,DM),group C(compound diabetes and periodontitis group,DM + CP).The Diabetes group was fed with high-fat and high-sugar diet for 8 to 10 weeks,and then multiple injected with streptozotocin in small dose on the condition of ice bath,dynamic monitoring of blood sugar and glycated hemoglobin values.The Periodontitis model was established using a method of periodontal ligation,select bilateral maxillary first and second molars,wounded 4-0 surgical suture in 0.2mm orthodontic ligation wire,ligation of the first and second molar on the condition of general anesthesia,and coating P.gingivalis suspension in periodontal.After 14 weeks randomly sclected 7 in each group to put to death.Then divided group C into 5,C1(natural process group),C2(initial therapy group),C3(initial therapy + antibiotics group),C4(tooth extraction group),C5(tooth extraction + antibiotics group),conduct corresponding oral intervention of each group,5 weeks after intervention,to be put to death all the rats and collect the bilateral carotid artery blood vessels.the jaw samples were subjected to methylene blue staining,observed periodontitis bone resorption.Collected bilateral carotid artery tissue samples,the left side of the real-time PCR to detect bacteria content,the right side of the staining to observed pathological changes Results:1.Methylene blue staining results showed that the alveolar bone absorption of composite model was the worst.Molar teeth gap in the first and second was widened,and the root furcation was exposure.About one week after STZ injected,the blood sugar in B and C rose sharply and volatility,With the typical clinical "three more than a little" symptoms,FPG ? 7.8 mmol/L,and(or)random blood sugar ? 17.8 mmol/L.About two-three weeks later keeped at a stable state of moderate hyperglycemia.4 weeks after modeling the HbA1 C level in B and C shows significantly higher than the normal control group,the difference was statistically significant(P < 0.01).2.Pathological results showed that group C was significantly more severe than the other,Thinning of vessel wall,broken or even disappeared elastic fibers,and no obvious smooth muscle cell were observed in Group C.The fluorescent quantitative PCR results showed that three groups were detected porphyromonas gingivalis DNA fragments m,group NC 42.86%,group DM 42.86%,group DM + CP 100%,DM and DM+CP compared with group NC showed higher bacteria content and the difference was statistically significant(P < 0.01).3.HbA1 C level after the intervention was higher than before.Variance analysis results showed no statistical significance about before and after the intervention.4.Pathological results in five weeks after intervention showed that the elastic fiber in group B is disorder with smooth muscle cell vacuolar changes;group C1 showed intimamedia layer thickening,several specimens appeared calcified clumps,smooth muscle cell cavity becomes more,and tunica intima damaged.Group C2 and C3 of intima-media layer showed uneven thickness,elastic fiber disordered,vacuolar changed more.Group C4 lesion was the most serious,all specimens appeared uneven thickness in vessel wall,elastic fiber fracture,calcification of intima-media layer was seriously,several specimens of calcified plaque were took off after dyeing.C5 compared with C4 was improved,but serious than the other,characterized by intima-media layer thickening,elastic fiber arrangement is disorder,smooth muscle cell degeneration,rare specimens intima-media layer can found calcified clumps.After the intervention Pg detection quantity is decreased obviously.others compared withC1,P < 0.01,was statistically significant.Conclusion:1.The experiment successfully established SD rat model of type 2 diabetes mellitus and periodontitis.2.porphyromonas gingivalis may participate in the process of diabetes vascular lesions.3.Periodontitis can promote diabetic rats alveolar bone resorption and the diabetic vascular complications.4.Oral intervention cannot improve blood sugar of diabetic periodontitis,but can effectively reduce Pg content of blood vessels.5.Tooth extraction with antibiotics measures has the best effect on reduce the content of Pg,then the initial therapy +antibiotic group,but its damage on artery is also the most.
