Objective: To investigate the correlation between the level of glycosylated hemoglobin(HbA1c)in type 2 diabetes mellitus and the parameters of anterior segment measured by Pentacam anterior segment analyzer.Methods: 54 patients(86 eyes)with type 2 diabetes who were hospitalized in ophthalmology department of our hospital from July 2018 to February 2019 were selected for cataract surgery.86 eyes were divided into three groups according to the level of HbA1c: group A,HbA1c<6.5%,28 eyes,including 18 males and 10 females;group B,6.5%?HbA1c?8%,36 eyes,including 14 males and 22 females;group C,HbA1c>8%,22 eyes,including 13 males and 9 females.Record sex,age,history of diabetes,intraocular pressure,visual acuity,refractive data,glycosylated hemoglobin and so on.The central corneal thickness(CCT),the thinnest corneal thickness(TCT),the upper,lower,nasal,temporal and average peripheral corneal thickness(PCT),anterior chamber volume(ACV),anterior chamber angle(ACA),and anterior chamber depth(ACD)were collected by Pentacam anterior segment analyzer.SPSS19.0 statistical software was used for data analysis and processing.Chi-square test,one-way ANOVA or non-parametric rank sum test were used to compare the parameters between the two groups.LSD method was used to compare the parameters between the two groups.Single-factor correlation analysis(Pearson or Spearman)was used to analyze the correlation between HbA1 c level and corneal thickness and anterior chamber parameters.Results: There was no significant difference in age,sex and course of disease among the three groups(P>0.05).There were significant differences in CCT,TCT,upper PCT,lower PCT,nasal PCT,temporal PCT,average PCT,ACV and ACD levels among the three groups(P<0.05).The levels of CCT,TCT,upper PCT,lower PCT,nasal PCT,temporal PCT and average PCT increased with the increase of HbA1 c level,while ACV and ACD decreased with the increase of HbA1 c level.Univariate correlation analysis showed that HbA1 c was positively correlated with CCT(r = 0.48,P<0.01),TCT(r = 0.51,P<0.01),lower PCT(r=0.54,P< 0.01),temporal PCT(r= 0.43,P<0.05),average PCT(r=0.41,P<0.05)in the HbA1c<6.5% group,There was no correlation between HbA1 c and parameters in 6.5%?HbA1c?8% group and HbA1c>8% group.Analysed by all the subjects,HbA1 c was positively correlated with CCT(r=0.45,P<0.01),TCT(r=0.45,P<0.01),upper PCT(r=0.38,P<0.01),lower PCT(r=0.51,P<0.01),nasal PCT(r=0.43,P<0.01),temporal PCT(r=0.47,P<0.01),average PCT(r=0.44,P<0.01).In the univariate correlation analysis of HbA1 c level with anterior chamber parameters in T2 DM,it was found that HbA1 c was negatively correlated with ACV(r=-0.34,P=0.03)?ACD(r=-0.40,P=0.04).Conclusion: 1.In type 2 diabetes mellitus,HbA1 c level is positively correlated with corneal thickness,and negatively correlated with anterior chamber depth and anterior chamber volume.2.Controlling the level of HbA1 c in type 2 diabetes mellitus can reduce or delay the occurrence of anterior segment complications in diabetes mellitus.
