ObjectivesTo study the patient satisfaction and influence factors on cataract day surgery.Finding the weakness in the process of medical service and offer the measures to improve patient experience and satisfaction of medical service.Explore the suitable procedure and management mode of day surgery for sustainable development of hospital.MethodsAll 602 patients of cataract day surgeries were reviewed from July to December,2016 in the hospital.A total 524 valid questionnaires were collected,the rate of effective was 97.7%.Collected the information of the population characteristic,the knowledge of day surgeries,the patient satisfaction of cataract day surgeries by questionnaire survey.SPSS23.0 software was used for statistical analysis,with descriptive statistics,chi square test and bivariate multiple logistic regression analysis for patient satisfaction and influence factors on cataract day surgery.The test level was at 0.05.Results1.The population characteristic of patients: A total 524 cases were collected including 214 males and 310 females.The average age of patients was 70.1 years,with minimum 30 y and maximum 88 y.2.The awareness rate of cataract day surgery:(1)By chi square test,there was the significant difference of the awareness rate of cataract day surgery between pre-and post-operation(X~2 =135.666 P<0.001).(2)The top three ways to understand the day surgery were from doctor(71%),relatives and friends(20%)and other patients during post-operative review(6.1%).(3)The top three reasons for day operation included reduction of waiting and treatment time(31.5%),gone home more earlier for rehabilitation and smaller effect on life(18.3%),and doctor recommendation(17.6%).(4)By chi square test,there was the significant difference of the awareness rate of cataract day surgery between male and female(X~2 =10.555,P=0.001).There was the significant difference in the distribution of the awareness rate of cataract day surgery among different occupation(X~2 =19.806,P=0.003),different income(X~2 =14.069,P=0.001),and different medical insurance mode(X~2 =14.140,P=0.003).3.Patient satisfaction survey of cataract surgery showed that: Overall satisfaction of hospital was 91.2%.Patient satisfaction on treatment process was 88.9%,on the quality of medical service 98.8%,doctor-patient communication 92.9%,medical ethics 97.5%,medical expenses 76.7% and hospital environment 92.3%.4.Bivariate multiple Logistic analysis results showed that degree of education,having or not past-operation history and degree of awareness in post-operation affected the overall satisfaction(P<0.05).In degree of education,if we choose junior college / college degree or above as control,patients having attended technical secondary school / high school and patients junior high school or below were more satisfied to medical service,with OR 3.771,2.546 respectively.The overall satisfaction in patients having past-operation history were 4.763 times than those who have not past-operation history.In awareness of day surgery among postoperative patients,awareness of day surgery affected the patient satisfaction significantly,with OR 0.411.In other words,patients with less awareness of day surgery were more satisfied with the day surgery than those with more awareness.Conclusions1.Overall satisfaction of cataract day surgery at a hospital in Shanxi was 91.2%.Patient satisfaction of treatment process and medical cost is relatively lower among six aspects.2.Among patients having day surgery,gender,education background,occupation,average monthly income,and medical insurance mode have affected the awareness.Education background,having or having not past-operation history have also affected the patient satisfaction.3.Suggested to strengthen the publicity of day surgery to increase public awareness of day surgery,adopt multiple channels to reduce medical costs and complete medical insurance policy.Optimize medical service process and change medical management mode,provide personalized service for different patients in order to improve the satisfaction rate of patients and boost the development of day surgery.
