Backgrounds China is one of the world’s3areas with highest prevalence rate of blindness.There is nearly7million blind ones, accounting for18%of the total of the whole world. Among them,50%is resulted from cataract. Operation treatment is considered as the most effective method to cure cataract.To improve the number of operation treatment and ensure its quality is the key to eliminate the blindness and visual impairment caused by cataract. At present, the pathogenesis of cataract is not very clear, and studies related to cataract operation service utilization are also very rare in china. This research is mainly to explore the related factors of cataract, and also analyze the obstacle factors of cataract operation service utilization,so as to provide the basis for cataract prevention strategy.Objectives To investigate the prevalence,surgery rate and also rate of surgery coverge of cataract,and analyze their influencing fatcors among adults aged50years and above in Tengzhou of Shandong province.Methods Cluster sampling was used in randomly selecting individuals aged50years and above in27basic sampling uints of Tengzhou city. During April to July,2008, the investigation was carried out.Before the field work,the smpling population were enumerated.Vision acuity was measured by logarithm visual chart. The prevalence, surgery rate and also rate of surgery coverge of cataract were calculated according to the standare used by Professor Zhao Jialiang of Xihe Hospital in the national epidemiological survey. The survey was preceded by a pilot study.The quality assurance were carried out before the field work,during the field work period,and after the field work.Results5557individuals were enumerated,4916individuals were examined. The response rate was88.46%.(1)Of4916subjects,1308were cataract patients, the prevalence rate was26.6%. Univariate analysis showed:prevalence rate of female was29.8%, and male was22.6%(P=0.000); the prevalence rate of50-59age group was6.6%, of the60-69age group was27.1%, of the70-79age group was58.1%, of80years of age and older group was78.5%(P=0.000); the prevalence rate of illiteracy group was33.4%, under primary school group was15.6%, primary school group was14.4%, the junior high school group was12.1%(P=0.000); prevalence rate among those in marriage was21.1%, single was53.3%(P=0.000); per capita income0-Yuan group was50.6%,1000-Yuan group was28.8%,3000-Yuan group was19.4%,5000-Yuan group was14.6%(P=0.000); prevalence rate among those who from low-income households was48.3%, non-low-income residents was25.8%(P=0.000); prevalence rate of self-rated health in good, normal,poor status was19.5%,31.2%,42.8%(P=0.000); prevalence of blindness and low vision relatives was34%, no the prevalence rate was26.1%(P=0.002); no smoking prevalence rate was24.4%, the prevalence of smokers was27.3%, before the absorption now or the prevalence rate was33.5%(P=0.004); never drinkers prevalence rate was17.3%, the prevalence rate of24.2%for the occasional drink, often drink patients Disease rate was29.4%, used to drink now or the prevalence rate was27%(P=0.000); taste light, medium, heavy salty prevalence were24.2%,27.6%,31.4%(P=0.000). Multivariate analysis showed, gender, age, marital status, education level, per capita income, self-rated health, there is no relative factors of blindness and low vision and drinking status were risk factors of cataract prevalence.(2)Of4916subjects,65underwent cataract surgery, the rate was 1.3%. Univariate analysis showed:rate of female was1.0%, and male was1.6%(P=0.048); the rate of50-59age group was0.0%, of the60-69age group was1.2%, of the70-79age group was3.7%, of80years of age and older group was4.2%(P=0.000); the rate of illiteracy group was1.7%, under primary school group was0.1%, primary school group was0.3%, the junior high school group was0.7%(P=0.034);the rate among those in marriage was0.8%, single was3.7%(P=0.000); per capita income0-Yuan group was2.8%,1000-Yuan group was1.5%,3000-Yuan group was0.7%,5000-Yuan group was0.7%(P=0.000); the rate among those who from low-income households was4.5%, non-low-income residents was1.2%(P=0.000);the rate of those who has health insurance was1.4%,of those who has no health insurance was0.4%(P=0.041);the rate of self-rated health in good, normal,poor status was1.0%,1.3%,3.4%(P=0.000); the rate of self-rated vision in good, normal,poor and blindness status was0.2%、1.1%、4.1%,9.7%(P=0.000);the rate of self-rated vision discomfort was2.2%,and those who had not was1.2%(P=0.017);the rate of those who thought cataract curable was1.8%,incurable was0.7%,and those who did not know was1.0%(P=0.024). Multivariate analysis showed, age, self-rated vision statue,the cognition on cataract and health insurance were factors related with rate of cataract surgery.(3)Of97who should undergo cataract surgery,35underwent cataract surgery, the rate of cataract surgery coverage was36.1%. Univariate analysis showed:the rate of female was35.6%, and male was37.5%(P=0.868); the rate of50-59age group was25.0%, of the60-69age group was45.0%, of the70-79age group was46.8%, of80years of age and older group was11.5%(P=0.018); the rate of illiteracy group was37.2%, under primary school group was50.0%, primary school group was0.0%, the junior high school group was33.3%(P=0.587);the rate among those in marriage was30.2%, single was40.7%(P=0.284); per capita income0-Yuan group was35.7%,1000-Yuan group was38.7%,3000-Yuan group was20.0%,5000-Yuan group was42.9%(P=0.683); the rate among those who from low-income households was55.6%, non-low-income residents was34.1%(P=0.202);the rate of those who has health insurance was41.3%,of those who has no health insurance was11.8%(P=0.022);the rate of self-rated health in good, normal,poor status was37.5%、33.3%、40.0%(P=0.857); the rate of self-rated vision discomfort was42.3%,and those who had not was33.8%(P=0.440);the rate of those who thought cataract curable was45.0%,incurable was25.0%,and those who did not know was31.1%(P=0.286)Conclusions(1) The prevalence rate of cataract among residents aged50years and above in Tengzhou city of Shandong Province was26.6%, rate of cataract surgery was1.3%, rate of cataract surgical coverage was36.1%;(2) the prevalence rate of cataract was related to factors including gender, age, marital status, education level, per capita income, self-rated health, blindness and low among relatives and drinking;(3)The rate of cataract surgery was related to factors including age, visual acuity, cognition on cataract treatment and health insurance;(4) The rate of cataract surgical coverage was related with factors including age,health insurance.
