Objective:The incidence of HLAP has been increasing in recent years.It has become the third most serious risk factor of acute pancreatitis in China after biliary and alcoholic diseases.Worldwide,the number of HLAP patients has increased year by year,and more and more attention has been paid to it in clinical work.In this paper,we compare the medical records of HLAP patients with the most common BAP patients in our country at present,and use statistical methods to get the results and explain the clinical features of HLAP.Methods:The object of study for acute pancreatitis diagnosis of the second hospital of Shanxi Medical University and the First Affiliated Hospital of Shanxi Medical University in January 2015-2017 year in January admitted to the medical records of the patients(AP),the exclusion criteria strictly choosing cases:(1)incomplete medical records;(2)the onset time of more than 48 h patients admitted to hospital;(3)no blood lipid the patients were admitted to 24h;(4)biliary acute pancreatitis complicated with hyperlipidemia patients;(5)for various reasons did not receive complete treatment offered to patients discharged due to.A total of 78 cases were selected,of which 20 patients with hyperlipidemic acute pancreatitis were treated as the observation group,and 58 patients with acute biliary pancreatitis with normal blood lipid as the control group.All patients were treated in accordance with the guidelines for the routine treatment of acute pancreatitis,including fasting,gastrointestinal decompression,pain,inhibit trypsin(treated by somatostatin and its analogues),anti infection,rehydration,nutrition support.The information included the general condition of the patients: age,gender,body mass index(body mass,index,BMI)and the incidence of fatty liver and diabetes;laboratory index: admission within 24(amylase,AMY)of serum amylase and urine amylase,serum lipase,serum triglyceride(triglyceride,TG),blood glucose,serum calcium and C-reaction protein;admission severity of 48 h type and Ranson score,CTSI score;treatment: surgery,intensive care,hospitalization days and recurrence after discharge.The two groups of data were analyzed and compared according to the above three aspects by statistical methods.Then we compared and analyzed all the indexes with statistical significance,and then expounded the clinical characteristics of hyperlipidemic acute pancreatitis.Results:There was no significant difference in sex ratio between HLAP and BAP in the two groups of patients as BMI,the average age of HLAP group(35.8±14.12)years old,average age of BAP group(50.6±17.31)years old,two compared with statistical significance,HLAP group,diabetes,fatty liver disease incidence was higher than that in group BAP(P < 0.05);Between the two groups of laboratory index contrast group HLAP serum amylase level average(545.17±335.78U/L)than group BAP(845.3±465.9U/L)(P < 0.05),compared with the average level of alt,HLAP(52.6±26.7U/L)was significantly lower than that of group BAP(198.7±143.8U/L)(P < 0.05);urine AMY,two group the serum lipase,serum calcium levels were not significantly abnormal(P > 0.05);The blood glucose level and C reactive protein in HLAP group were higher than those in group BAP(P < 0.05).Compared with the group,the Ranson score and the incidence of severe patients in the HLAP group were higher than those in the BAP group(P < 0.05),and the average hospitalization time was not significantly different(P > 0.05).Conclusion:The age of onset of hyperlipidemic acute pancreatitis is lower than that of acute biliary pancreatitis.HLAP patients are more common in young and middle-aged patients with diabetes and fatty liver.Serum amylase levels in some patients with HLAP may be normal or mildly elevated.The average value of C-reactive protein and blood glucose was higher than that of BAP group,Alanine aminotransferase was generally lower than BAP patients.HLAP patients are prone to develop severe and severe acute pancreatitis.HLAP patients often require intensive care.The recurrence rate of patients with HLAP was high.
