Objective:To compare and analyze the effects of two kinds of patellar movement on the recovery of knee function after total knee arthroplasty(TKA).Methods:From Mar 2013 to Sep 2015,60 patients Severe knee Osteoarthritiss performed primary unilateral TKA were retrospective studied.35 patients underwent patellar eversion and 25 patients received lateral retraction.Two groups were compared according to volume of operation time,postoperative length of stay,the time to achieving strait leg raise(SLR time),Insall-Salvati ratio at 1 year postoperative,Range of motion(ROM),visual analogue scale(VAS),Knee Surgery Society(KSS)knee Score and knee function score at preoperative,postoperative 1week,6 weeks,3 months and 1 year.Results:Total 60 patients were followed up.The patellar eversion group obtained mean13(range 12-14 months)followed up.The patellar lateral retraction obtained mean13.3(range 12-14 months)followed up.In the patellar eversion group,there were 1patient with slight avulsion of the patellar tendon,while there were no patient in the patellar lateral retraction group.The operation time of the patellar lateral retraction group increased more than that of the patellar eversion group,but the difference was not statistically significant(P>0.05).To the SLR time,the patellar lateral retraction group earlier than patellar eversion group,and the difference was statistically significant(P<0.05).But there were no significant differences in the postoperative length of stay and Insall-Salvati ratio at 1 year postoperative(P>0.05).The VAS score,ROM score,KSS knee score and functional score at different time points were significantly improved,but there was no significant difference between the two groups(P>0.05).Conclusion:The patellar lateral retraction increased postoperative time of active straight leg raising,indicating patellar lateral retraction obtain early functional recovery of knee joint after operation,but the long-term recovery of knee joint function had no difference between the two groups,patellar eversion and patellar lateral retraction can also obtain good long-term clinical effect.But because the evaluation method will bring some error to the result,a large number of random tests will be needed to verify the conclusion.
