| Objective:To observe the effectiveness of MFBO combined with TCM fomenting therapy for the treatment of protrusion of lumbar disc herniation through a clinical research.Methods:1.Research objects: Selected 60 patients with LDH from Shanxi HuaKang Spondylopathy Hospital during May to Dec,2016 as the research sample data according to inclusion and exclusion standard method.2.Research strategy: The 60 patients was randomly evenly divided into three different treatment groups---Massage for Bone Orthopedics(MFBO)combined with TCM fomenting Therapy group,Massage for Bone Orthopedics group,TCM Fomenting Therapy group.Patients in each group were all treated for a month in every three days.Of the three treatment methods,MFBO combined with TCM fomenting therapy is the main study object and other two methods are for comparisons.Patients in MFBO combined with TCM fomenting therapy group were treated by MFBO together with TCM fomenting therapy;Patients in MFBO group were only treated by MFBO therapy;Patients in TCM fomenting therapy group were only treated by TCM fomenting therapy.3.Outcome measures: general information of patients was collected before treatment.In order to observe the pain condition and the comprehensive recovery status of patients,VAS pain grade,JOA scores,and response evaluation were collected before treatment,and 48 hrs,13 days and 28 days after the treatment.4.Therapeutic evaluation: The VAS pain score illustrated by the book---The evaluation standard of clinical study on orthopaedics(2005)---was used as a criteria to assess the pain of patients;JOA was used as a standard evaluation on assessing the comprehensive situation of a LDH patient.And set the healing efficacy criteria based on standard “Criteria of diagnosis and therapeutic effect of diseases and syndromes in traditional Chinese medicine” published in 2012.5.Statistic test: F-test,Chi-squared test were used to analyze the sample data via SPSS17.0;Confidence interval is expressed as mean ± standard deviation;P-value smaller than 0.05 indicates that the null hypothesis of the sample is true and the sample is unbiased and meaningful.Results:1.General comparisonAnalysis of variance(ANOVA)was used to analyze the gender,age,BMI,disease course and other general data of the three groups of patients.The results showed that there was no significant difference between the three groups(P> 0.05),which could exclude the deviation caused by sex,age,BMI and course of disease,suggesting that the three groups were comparable.2.Comparison of efficacy scores1)VAS pain gradeThe VAS pain scores of three groups were all reduced after treatments.Before treatments,the confidence interval of the comprehensive treatment group,the MFBO group and TCM fomenting group was 7.8 ± 1.07,8.1 ± 0.79 and 8.2 ± 0.81 respectively,and p-value of the test was equal to 0.316 before treatments(greater than 0.05),which was not statistically significant.After 48-hours treatments,the confidence interval of the comprehensive treatment group,the MFBO group and the TCM fomenting therapy group decreased to 5.7 ± 0.99,6.2 ± 0.95 and 6.6 ± 0.99 respectively.In this test,the calculated p-value is equal to 0.018(smaller than 0.05),which is statistically significant.After13-days treatments,the confidence interval of the comprehensive treatment group,the MFBO group and the TCM fomenting therapy group reduced to 1.2 ± 1.14,1.9 ± 1.74,VI and 2.85 ± 2.11 respectively,and p-value was equal to 0.010(smaller than 0.05),which was statistically significant.2)JOA scoresThe average JOA scores of the three groups were improved.Before treatments,the confidence interval of the comprehensive treatment group,the MFBO group and TCM fomenting group was 8.2 ± 1.14,8.0 ± 1.49 and 7.5 ± 1.28 respectively,and the calculated P = 0.266 > 0.05,which was not statistically significant.After 48-hours treatments,the confidence interval of the comprehensive treatment group,the MFBO group and TCM fomenting group was 8.6 ± 1.6,8.2 ± 1.14,and 7.8 ± 1.32 respectively.After 13-days treatments,the confidence interval of the comprehensive treatment group,the MFBO group and TCM fomenting group was 21.2±2.4,19.9±3.69,and 18.2±4.73 respectively,and the calculated P=0.046 < 0.05,which was statistically significant.After 28-days treatments,the confidence interval of JOA scores test of the comprehensive treatment group,the MFBO group and TCM fomenting group was 25.1 ± 1.48,23.4 ± 4.01,and21.7 ± 5.20 respectively,and the calculated P=0.028<0.05,which was statistically significant.3.Efficiency comparisonAfter a course of treatment,the total rate of efficiency of the comprehensive treatment group,the MFBO group and TCM fomenting group was 95%,85% and 70%respectively.As indicated in the above results,the treatment effect MFBO combined with fomenting therapy is better than that of the MFBO only therapy and TCM fomenting only therapy.As a result,the most effective way to cure LDH is MFBO combined with TCM fomenting therapy.Conclusion:The research shows that the clinical efficacy of MFBO combined with TCM fomenting therapy is significantly better than that of MFBO only and TCM fomenting only therapy,and MFBO only therapy has a better effect than that of TCM fomenting only therapy.According to the evaluated VAS pain score,it's clear that the comprehensive therapy has a better effect in reliefing the pain of LDH patients than that of MFBO only therapy,and hence than that of TCM fomenting therapy.As indicated from the evaluated JOA score,the LDH comprehensive recovery situation result for thecomprehensive therapy reviewed a better curing effect than that of MFBO only therapy,and hence than that of TCM fomenting therapy.In addition,no adverse reactions of one patient were reported and discovered during these three treatments.In conclusion,the application of the comprehensive treatment therapy was an effective method for the treatment of LDH.Furthermore,which had obvious curative effect with simple operation,low risk,and it was easy to be accepted by the majority of patients. |
