ObjectiveTo investigate and analyze the current situation of self-management behaviors among Hui nationality patients with type 2 diabetes mellitus and to based on trans-cultural nursing theory to evaluate the application effect of management by problem approach in it.MethodsManagement by problem approach combined with cultural backgrounds of type 2 diabetic patients of Hui settlement was applied to find out the existing or potential problems in self-management behaviors of Hui diabetic patients.Then we formulate and implement intervention plans.A total of 76 subjects were chosen to engage in the study by convenience sampling method,which came from the endocrinology department of Wuzhong people's hospital and lived in Hui communities.The samples were divided into two subgroups.Control group(n=37)received usual health education,while intervention group(n=37)performed usual health education as well as the intervention plans that based on management by problem for six months.The scores of self-management behaviors,hemoglobin A1c(HbA1c)and body mass index(BMI)in T2DM patients were compared and analyzed before and after the intervention.Results1.In this study,totally 74 T2DM patients including 37 in intervention group and 37 in control group were included.Among them 35 subjects were male,39 subjects were female,the average age was(64.12±7.84)years old.After the comparison,two groups had no significant difference in the general data(P>0.05),which indicated that two groups of T2DM patients with baseline data were comparable.2.After the intervention,the score of self-management behaviors of the intervention group was 108.14±8.11,the control group was 89.76±4.96,their difference was significant(P<0.01).3.For the intervention group,the score of self-management behaviors before the experiment was higher than after with significant difference(P<0.01).4.There are differences among the scores of self-management behaviors of two groups during four intervention period(Before the intervention,the day of discharge,at 3 months and 6 months)with statistically significant(P<0.01).5.After the intervention,the hemoglobin A1c(HbA1c)of the intervention group was significantly lower than that of the control group(P<0.01).6.After the intervention,body mass index(BMI)of the intervention group was significantly lower than that of the control group(P<0.05).Conclusions1.The application of trans-cultural nursing can be used to find out and solve the existing or potential problems in self-management behaviors of T2DM patients,from the perspective of culture.2.The application of trans-cultural nursing combined with management by problem approach has played a positive role in enhancing the self-management behaviors of T2DM patients.
