ObjectiveTo explore the effect of cooperative nursing mode based on characteristic health education of TCM on self-management ability of type 2 diabetes mellitus with Yin deficiency and heat.Methods1.From April to September 2017,84 patients with T2 DM with Yin deficiency and heat were investigated in Department of Endocrinology,Henan traditional Chinese Medicine Hospital.The subjects were divided into two groups by random digital table.According to the order of admission,the first 42 cases were the experimental group and the latter 42 cases as the control group.The control group was given routine nursing of admission assessment and registration and health education with TCM characteristics.The experimental group adopted the cooperative nursing model on the basis of the control group,and set up a multi-disciplinary collaborative nursing intervention group during the hospital.Before discharge,the patients' self-care ability and bedside guidance were evaluated by the group of nursing intervention together.The patients were treated by telephone and We-Chat after discharge.2.After 3 months of intervention,the self-management behavior of type 2 diabetes mellitus,the specificity of quality of life,the knowledge of diabetes mellitus and the related physiological indexes were evaluated respectively in the two groups.3.The counting data are expressed by frequency or rate,compared by chi-square test,and statistical analysis is carried out by two independent samples t test,rank sum test,repeated measurement variance analysis and so on.Results1.The comparison of the total score of diabetes knowledge assessment scale showed that:The total score before intervention was 11.00±1.71 in the trial group and11.05±1.29 in the control group.The results showed that there was no statistical significance between the two groups before the intervention(t=0.144,P=0.886);The total score after intervention was 16.76±1.86 in the experimental group and 10.86±1.96 in the control group.The comparison showed that the score of the experimental group after intervention was higher than that of the control group,and the difference was statistically significant(t=14.176,P<0.001).2.Before and after intervention,the total score of self-management behavior scale was 60.57±5.05 in the trial group and 60.74±6.70 in the control group.The results showed that there was no significant difference in the total score of the two groups before the intervention.The comparison showed that there was no significant difference between the two groups(P>0.05).After intervention,the total score of the scale was75.62±6.03 in the trial group and 60.26±8.33 in the control group.The comparison showed that the total score of the test group was higher than that of the control group,and the difference was statistically significant(t=9.682,P<0.001).The scores of each dimension in the trial group were higher than those in the control group(P<0.05).3.Before and after intervention,the scores of DSQL in the two groups were 62.17±5.21 and 62.26±6.78,respectively.The comparison showed that there was no significant difference in the total scores of DSQL between the two groups before and after the intervention,but there was no significant difference between the two groups in the total scores of DSQL before and after intervention,but there was no significant difference in the total scores between the two groups before and after the intervention(t=0.072,P=0.943).The comparison of each dimension showed that there was no significant difference between groups(P>0.05).After intervention,the average total score of the trial group was 53.64±5.06,and that of the control group was63.02±6.09.The comparison showed that the total score of DSQL in the trial group was lower than that in the control group,and the difference was statistically significant(t=7.678,P<0.001).4.A 3 month intervention can have a positive effect on the clinical indicators of patients.Through the intervention group,before and after the intervention,the patients in the control group compared with themselves before and after the intervention.The use of collaborative nursing mode can change the FPG,2hFBG,HbA1 c of patients,the difference is statistically significant(P<0.001).Conclusion1.The cooperative nursing model can help patients to improve theirself-management ability,quality of life and blood sugar control.2.The intervention effect of the cooperative nursing model based on the characteristic health education of traditional Chinese medicine is obviously better than that of the routine education and nursing.It is worth to adhere to and improve further,and to provide the basis for the management mode of chronic disease of traditional Chinese medicine.
