Objective We used the cross-sectional study to investigate pregnant depression and its factors at prenatal,3 days and 42 days of postnatal stages;and to analysis the correlation between orphanin FQ(OFQ)and postpartum depression.Methods(1)The current status of maternal depressionWe selected 140 pregnant women from Yinchuan maternal and child health hospital and 60 pregnant women from Zhongwei traditional Chinese medicine hospital.A total of 200 pregnant women were enrolled in the study.The prenatal of depressive symptoms was assessed using the Self-rating Depression Scale(SDS),and postpartum depression was assessed using the Edinburg Postnatal Depression Scale(EPDS).The 42 days of depression evaluation was followed by cellphone,other questionnaires were completed by pregnant women themselves.(2)The correlation between OFQ and postpartum depressionWe collected 176 blood samples from inpatient department of the two hospitals base on voluntary participation.The serum OFQ levels were detected by ELISA in postpartum group and control group,analyzed the significant difference between two groups.Results(1)The status of maternal depressionThe prevalence rate of prenatal depression was 20.0%;the average score of SDS was significantly higher than that of the national norms(43.24±7.46 vs 41.88±10.57,t = 2.584,P < 0.05);the average scores of age ?30y group was(43.98±6.93),and that of <30y group was(42.80±7.76),there was not statistically significant differences between the two groups(t = 1.179,P > 0.05).The prevalence rate of the postpartum depression within 3 days was 26.0%,the average scores of EPDS was(9.18±4.20);the EPDS scores of age ?30y group was(9.39±4.36),and that of <30y group was(8.81±3.93),the differences was no statistically significant between the two groups(t = 0.888,P > 0.05).The prevalence rate of postpartum depression within 42 days was 15.5%,the average score of EPDS was(8.08±3.95);the EPDS scores of age ?30y group was(8.14±4.17),and that of <30y group was(7.63±3.41),the differences was no statistically significant between the two groups(t = 1.134,P > 0.05).Multivariate logistic regression analysis showed that the postpartum depression were correlated with parity(OR = 0.084,95%CI: 0.009~0.748),couple relationship(OR = 6.655,95%CI: 1.395~31.743),abortion history(OR = 5.013,95%CI: 1.637~15.354),pregnancy complications(OR = 10.440,95%CI: 2.829~38.528),mother-in-law relationship(OR = 12.640,95%CI: 2.108~75.802),sleep quality(OR = 2.701,95%CI: 1.088~6.701),domestic violence(OR = 39.305,95%CI: 1.310~1179.219).(2)The correlation between OFQ and postpartum depression.The average level of OFQ was(27.11±14.24)pg/ml for all participatens.The OFQ content of prenatal depression was obviously higher than the control group(32.11±14.56 pg/ml vs 25.84±13.61 pg/ml,t =-2.365,P = 0.019);the OFQ content of postpartum depression group was also obviously higher than the control group(45.61±16.56 pg/ml vs 23.07±10.06 pg/ml,t = 93.587,P < 0.001).Pearson analysis showed that the scores of EPDS was positively correlated with serum OFQ(r = 0.570,P < 0.001).ConclusionThe status of prenatal depression was higher than the national norms of Chinese population.The prevalence of depression was the highest at 3 days postpartum,followed by prenatal and 42 days postpartum.The risk factors of postpartum depression were poor couple relationship,poor mother-in-law,worried about child's health,poor sleep quality,abortion history,pregnancy complications and domestic violence.Serum OFQ was positively correlated with scores of EPDS,which prompted serum OFQ may be correlated with postpartum depression.
