Objectives Chitinase-3-1ike-1 protein(YKL-40)is a new inflammatory factor which was discovered in recent years.YKL-40 plays an important role in the development of inflammation,vascular disease and atherosclerosis.The purpose of this observation is to discuss the correlation between serum YKL-40 level and diabetic foot.Methods According to the diagnostic criteria of the WHO diabetes in 1999,from March2015 to October 2016,a total of 121 diabetics who hospitalized in the department of endocrinology of Kailuan General Hospital Affiliated to North China University of Science and Technology were evaluated in our study.All subjects were divided into diabetic foot group(DF group,69 cases)and type 2 diabetes mellitus group(DM group,52cases).According to the presence of ischemia in the foot,DF group was divided into two subgroups: ischemic group was 36 cases and non-ischemic group was 33 cases.All individuals' general clinical data were recorded.And the Wagner classification of the patients in the DF group was recorded.The serum YKL-40 concentration of all subjects was detected by Enzyme-linked immunosorbent assay(ELISA).All the subjects were measured by biochemical indexes,high-sensitivity C-reactive protein(hs-CRP)and glycosylated hemoglobin(Hb A1c).The difference of serum YKL-40 in each group was analyzed.And the relationship between serum YKL-40 level and diabetic foot was analyzed.Results 1 Compared with DM group,the levels of course,fasting plasma glucose(FPG),hs-CRP and serum YKL-40 in the DF group were significantly higher(P<0.05),the difference were statistically significant;the levels of hemoglobin(Hb)and bilirubin in the DF group were significantly decreased(P<0.05),the difference was statistically significant.When diabetes patients' serum YKL-40 level greater than 110.16ng/ml,they had a relative risk(OR value)for 13.537.Which showed that the risk of suffering from DF was 13.537 times when the concentration of serum YKL-40 in patients with DM was above110.16ng/ml.Whether diabetes with DF or not as the dependent variable,the significant difference indexes between the DF group and the DM group were analyzed viewing as covariate into the multivariate Logistic regression model.Logistic regression analysis showed that YKL-40 was the risk factor of diabetic foot.2 The comparison of ischemic group and non-ischemic group: compared with non-ischemic group,the course,FPG,hsCRP and serum YKL-40 level in ischemic group were significantly higher,the difference was statistically significant(P<0.05);the levels of Hb,bilirubin and HDL in ischemic group were significantly decreased,the difference was statistically significant(P<0.05);When diabetic foot patients' serum YKL-40 level greater than 123.90ng/ml,they had a relative risk(OR value)for 3.538.Which showed that the risk of suffering from ischemic diabetic foot was 3.538 times when the concentration of serum YKL-40 in patients with diabetic foot was above 123.90ng/ml.Whether diabetic foot with ischemia or not as the dependent variable,the significant difference indexes between the ischemic group and the non-ischemic group were analyzed viewing as covariate into the multivariate Logistic regression model.Logistic regression analysis showed that YKL-40 was the risk factor of ischemic diabetic foot.Conclusions 1 There is a positive correlation between serum YKL-40 and diabetic foot.And serum YKL-40 is involved in the occurrence and development of diabetic foot.2There is a positive correlation between serum YKL-40 and the degree of ischemia of diabetic foot.
