The Current Situation And Influential Factors Of Elderly's Frailty In Shijiazhuang Areas

Objectives 1 To translate the English version of the comprehensive frailty assessment instrument into Chinese.2 Evaluate the reliability and validity of the Chinese version of The CFAI.3 Applied the Chinese version of The CFAI to investigate the current situation of the aged people frailty in Shijiazhuang Community.4 Analyze the influencing factors of aged people frailty and to provide a theoretical basis for the development of effective community intervention measures on the frailty.Methods 1 According to the Brislin Translation Mode,The CFAI was translated into Chinese.2 A questionnaire survey was conducted in 200 Community aged people of Shijiazhuang from May 2015 to July 2015 using a convenience sampling method,Survey content as follows: The general demographic information(gender,age,education status,marital status,living situation,et al),The CFAI,The WHOQOL_BREF,30 patients in them were evaluated again in one weeks.All results after reviewing,the database established by using excel,the data was analyzed using the SPSS 17.0 software package.Count data using frequency was described,quantitative data using(`x±s)was described,The internal consistency of the scale using Cronbach's ? alpha coefficient to evaluate,The test-retest reliability and criterion-related validation of the scale using pearson correlation coefficient to evaluate,The content validity of the scale using expert evaluation method to evaluate,The structure validity of the scale using exploratory factor analysis to evaluate.3From August 2015 to July 2016,a multistage stratified sampling method was used to collect 3125 elderly people aged 60 years or over in a certain community in Shijiazhuang City as survey subjects and Conducted a questionnaire survey.The questionnaire content as follows: The general demographic information(gender,age,education status,marital status,income,balance of payments situation,living situation,et al),The health status(sight,hearing,chronic diseases,types of oral drugs,sleep quality,et al),The habit and customs(drinking,brushing habits,et al),The oral health(tooth loss,tooth function,toothache,et al),The family situation(family relationship,family status,number of children,children living condition,et al),The CFAI,ADL and The HPLP.All results after reviewing,the database established by using Excel,The data was analyzed using the SPSS17.0 software package.Count data using frequency was described,quantitative data using((?)±s)was described,Single factors quantitative data using one-way ANOVA or independent-samples t test,the factors of univariate analysis with statistical significance was incorporated into the multivariate analysis,the influence factors of elderly's frailty was screened.Results 1 A total 23 items in the Chinese version of The CFAI,including four dimensions: physical frailty(4~12 scores),psychological frailty(8~35 scores),social frailty(3~25 scores),environment frailty(5~25 scores),the total scores of The CFAI is ranging from 20~97.20 scores demonstrated that there was no frailty in aged people,97 scores demonstrated that there was very frailty in aged people and the higher scores says the more frailty.2 The Cronbach's ? alpha coefficient of The CFAI was 0.837;The intra-class correlation coefficients(ICC)of The CFAI was 0.604~0.941;the item-level CVI(ICVI)>0.83,scale-level CVI(S-CVI)=0.782;six common factors extracted by the exploratory factor analysis can explaining 64.052% of the variance of frailty;The score of the Chinese version of The CFAI and The score of WHOQOL_BREF showed a median negative correlation(r=-0.764,P<0.00).3 The current situation of frailty in the aged people in Shijiazhuang Community: The total score of frailty was(41.01±10.64),The average score of physical frailty was(1.28±0.556),The average score of psychological frailty was(1.88±0.669),The average score of social frailty was(2.09±0.529),The average score of environment frailty was(1.67±0.936),Frailty in the aged people presented the characteristics of "the highest level of social frailty",the elderly's frailty degree were increasing with age.4 Single factor analysis of elderly's frailty in community: The total scores of frailty were significant in age,education status,marital status,living situation,income,balance of payments situation,chronic diseases,types of oral drugs,sleep quality,ADL,drinking,brushing habits,HPLP,tooth loss,tooth function,toothache,living room light,community facilities,facility use,family relationship,family status,number of children and children living conditions(P<0.05).5 Multiple factors analysis of elderly's frailty in community: The main influential factors arranged from high to low of elderly's frailty were: HPLP,ADL,tooth function,education status,marital status,balance of payments situation,family relationship,community facilities,age,family status,chronic diseases,sleep quality,tooth loss,living situation,brushing habits.Conclusions 1 The Chinese version of The CFAI in agreement with the source scale,it has a good reliability and validity and could be used evaluate the elderly's frailty in community.2 The total score of elderly's frailty in Shijiazhuang community was(41.01±10.64)and presented the characteristics of "The highest level of social frailty",the elderly's frailty degree were increasing with age.3 Multiple factors analysis showed that the factors associated with the total score of elderly's frailty were: HPLP,ADL,tooth function,education status,marital status,balance of payments situation,family relationship,community facilities,age,family status,chronic diseases,sleep quality,tooth loss,Living situation,brushing habits.
