Objective: To evaluate the preliminary clinical value of C-arm CT-guided percutaneous transthoracic biopsy of small(? 10mm)pulmonary nodules.Materials and Methods:33 patients,a total of 34 small pulmonary nodules(diameter ? 10mm)under C-arm CT-guided percutaneous transthoracic biopsies.The mean nodule size was 8.8±1.1mm in diameter.17 cases were male and 16 female,mean age 61.3 years old.Patients were enrolled in guided by C-arm CT Percutaneous lung biopsy(PTNB).To record the puncture depth of each nodule,operative time and radiation doses.Technical success rate,diagnostic accuracy,sensitivity,specificity,positive predictive value,negative predictive value and incidence of complications were also evaluated.Results: The biopsy results in 34 small pulmonary nodules:22malignant lesions(64.7%),benign lesions in 11(32.4%),a pathological undetermined nodules(2.9%).Results the mean depth of pleural with the coaxial needle,biopsies,operative time,and coaxial introducer dwelling time and estimated radiation exposure during PTNBs were 27.8mm±13.4,3.13±0.51,12.1min±3.2,and 8.1min±2.3 and 4.8±1.8mSv.There were 11cases(32.4%)with complications,including 6 cases(17.6%)with pneumothorax,1 case with subcutaneous emphysema(2.9%),4 cases bleeding(11.8%),2 cases had clinical symptom complication,the incidence about 5.9%(pneumothorax and hemoptysis in 1 case),The remaining patients are trace needle bleeding and trace to a small amount of pneumothorax,were not special treatment.Conclusion: The C-arm CT combine real-time fluoroscopy and CTfeatures that will significantly overcome the lack of fluoroscopy and CT-guided percutaneous transthoracic biopsy,achieve real-time monitoring,precise positioning,rapid biopsy to reduce the radiation damage,improve biopsy accuracy,shorter operative time and reduce complications.Conclusion C-arm CT virtual-navigation-guided PTNB is a highly accurate and safe diagnostic method for small(?1 cm)nodules.
