| Since the year of 1992,family doctor system was established in Hungary.“family doctor”became the main provider of community medical services among community health system,meanwhile,compliance strictly with first visit in community and reasonable referral system throughout the process of implementation of the family doctor system.In China,“Guidance to State Council on the Establishment of General Practitioner System” was implement in 2011,importantly,the family doctor system to declare in the national health care strategy,however,primary health care lacks of function since advantages of resources concentrated in lots of large and medium-sized hospitals,whilst,health care resources didn’t applied well even health care with low efficiency.Therefore,it is valuable to do research on Hungarian family doctor.So,it has great practical significances and value to develop family doctor system in China,meanwhile,good for the direction of medical reform and avoid detours.Furthermore,it also explores the health care payment methods matched the family doctor system so that it could relieves the medical burden for residents and constantly improves the family doctor system in China.This paper starts with Marxist demand theory,stakeholder theory and incentive theory.First of all,introduce Hungarian family doctor,including the process of family doctor,the implementation principle on “focus on the benefit of residents”,”establishes on’biological-psychological’ medical model”,”implement on first diagnosis and two-way referral system”,moreover on health service framework and the direction of family doctor development.In addition,put forwards to the capitation health care payment mechanism and incentives on salaries and training.Secondly,it explores the effectiveness of the Hungarian family doctor system and the existence of health care informal payment,following,from the perspective of social-culture,legal-morality and economic efficiency to analyzes the reasons for the existence of informal payment.Thirdly,describe present situation in China which including the training of family doctor totally different from specialists,the underdevelopment referral mechanism,lack conscious on community residents,imbalance between supply and demand of family doctors even the shortage of professional training.Finally,puts forward some suggestions from Hungarian health care so that enlighten on the development of family doctor in China.For example,establishing the referral mechanism,strengthening community residents’ awareness,balancing the supply and demand of family doctors,building up incentive mechanism,avoiding medical informal payment.As the breakthrough point on Hungarian family doctor system establishment and development process with the history and development process so as to inspire the family doctor system,to some extent,provide theoretical support for the development on family doctor in China. |
