| The family doctor system is an important part of China’s medical and health system reform and an important cornerstone of the realization of the "Healthy China" strategy.The “doctor-patient trust” relationship under the family doctor system is the key to ensuring the long-term implementation of the system.Based on the perspective of "doctor-patient trust",the in-depth study of the optimization of the family doctor system is of great value to the rational allocation of medical resources and the healthy operation of the family doctor system.Based on the perspective of "doctor-patient trust",the thesis focuses on the family doctor system,using public product theory,symbol interaction theory,and trust theory as the theoretical framework,using mathematical models of analytic hierarchy process and empirical investigation methods to conduct research from five dimensions.First,on the basis of analyzing the connotation characteristics of the family doctor system,explain that the functional positioning of family doctors is the combination of health services and medical services,with health services as the mainstay.Second,family doctors and service targets should be a kind of "trust" Relationship or "acquaintance" relationship is a special kind of "doctor-patient" relationship,which is specifically manifested in the subject characteristics and interactive characteristics of the "doctor-patient" relationship.Third,the questionnaires and interviews are used to demonstrate to Shanghai residents and family doctors.The investigation shows that the number of people signing up for the family doctor system has increased,the relationship between family doctors and "patients" has improved,the residents’ trust in family doctors needs to be improved,and the service quality of family doctors is not satisfactory.Fourth,use the analytic hierarchy process to study the four major factors that affect the operation of the family doctor system under the perspective of "doctor-patient trust",including personal characteristics of residents,residents’ perceptions,attitudes and capabilities of family doctors,and medical services in community health institutions quality,and then analyze the bottlenecks in the operation of the family doctor system,that is,biased perceptions of residents and family doctors,family doctor services supply-demand structure mismatch,insufficient supply capacity of family doctors,and low enthusiasm of family doctors.Fifth,based on the perspective of trust theory,this paper proposes an optimization path of family doctor system from the perspective of improving interpersonal trust and system trust,and clarifies the goal of system optimization,promote the transformation of institutional functions,and put forward corresponding policy recommendations.The paper concludes that family doctors and residents have a special "doctor-patient" relationship and rely more on "doctor-patient trust." The inaccurate positioning of the family doctor system has led to a lack of trust between doctors and patients.With health services,health management,and public health services as the goal-oriented optimization of the family doctor system,enhance the family doctor’s service capabilities,optimize the family doctor system service content,and solve the problem of information asymmetry between "doctors and patients",thereby breaking interpersonal trust and The system of trust barriers promotes the healthy operation of the family doctor system and promotes the development of the "doctor-patient trust" relationship. |
