Investigation And Study On Family Doctor Service Under The Hierarchical Medical System

In 2011,in response to the reform of the national medical and health system and strengthening the capacity of community-level medical and health service institutions,Beijing began to pilot and promote family doctor services.Changping district of Beijing,as a pilot area,has developed well in publicity and promotion,service and other aspects and is more representative.With the method of random sampling,this study selected the four community health service centers as sample,with convenient sample of 260,the questionnaire survey to the family doctor service not subscribers by purposive sampling to 53 family doctor service subscribers and six family doctor interview investigation.Focus on the main conclusions in the process of research,this paper commissioned-agent theory and the theory of moral hazard,explores the Changping district of Beijing in the process of developing the family doctor service problems: first,from the point of research results,Changping district of Beijing family doctor service is still in the exploration period,how to promote community residents to participate in the family doctor service and how to motivate the family doctor service initiative,creativity is key.Second,trust issues arise in the principal-agent relationship between contracted residents and family doctors.Third,under the background of graded diagnosis and treatment,the role of grass-roots community health service institutions has not been fully played,and the road of realizing the pattern of graded medical treatment and orderly referral has been slow to develop.In this regard,the following Suggestions are made: 1.Expand publicity,highlight key points and implement personalized contract services.2.Pay attention to the training of general practitioners and improve the evaluation and supervision system.3.Innovative family doctor management mode.This includes expanding family physician membership and creating a family physician service network.4.Multi-party participation and coordination.It includes the cooperation between the government,the news media industry and the relevant medical and health service providers.
