ObjectivePostpartum urinary retention is a common obstetric complications which means voiding dysfunction or can not emptying the bladder completely.if this phenomenon is neglected and diagnosis and treatment is delayed,it will seriously affect the process of postpartum rehabilitation.The aim of the study is to identity the risk factors for PUR and find the independent delivery risk factors by using multivariate regression analysis.Of 378 women who had postpartum urinary retention after vaginal delivery in Beijing Obstetrics and Gynecology Hospital during January 2015 and December2015 were cases,and randomly selected 400 women as controls who were vaginal delivery and did not have PUR.Logistic regression analysis identity the independent risk factors for PUR to provide reference for the prevention of PUR.Methods378 women who suffered from PUR after vaginal delivery in Beijing Obstetrics and Gynecology Hospital from January 2015 to December 2015 were selected as cases and 400 women who did not have PUR at the same times and selected by randomly as controls.Collect the number of related factors,including maternal age,prenatal weight,diabetes,hypertension,the using of magnesium sulfate,analgesia,labor intervention(artificial rupture of membrane,using oxytocin for strengthen the uterine contraction,using Pethidine Hydrochloride(dolantin)for labor rest),intrapartum complications(laceration of cervix,hematoma of vaginal wall),vaginal delivery mode(natural delivery,instrumental delivery,episiotomy),birth weight ofnewborn.the investigation and analysis of records.Find the differences between the two groups datas by analyzing the collected data in statistics method.The data were analyzed by SPSS statistical software.The single factor analysis was performed by chi square test and the independent risk factors was performed by multivariate Logistic regression analysis,P?0.05 mesns the statistical analysis have significance.ResultsThere are 10078 cases of delivery in Beijing Obstetrics and Gynecology Hospital in January 2015 –December 2015 period,408 cases of postpartum urinary retention,the incidence of postpartum urinary retention rate was 4.05%,there were378 PUR women in the 7992 cases who were vaginal delivery,the incidence rate was4.73%.According to Univariate analysis,the age of pregnancy,prenatal maternal body weight,dibetes,hypertension,the use of Magnesium Sulfate,artificial rupture of membranes,using oxytocin for strengthen the uterine contraction,using Pethidine Hydrochloride(dolantin)for labor rest,cervical laceration,vaginal wall hematoma,birth weigh of newborn had no statistically significant difference between cases group and control group.In terms of forceps delivery,analgesia,episiotomy,manual rotation of fetal head and the time of second stage labor had significant difference between the case group and the control group.The length of hospitalizationin of the case group(5.335±2.16))was longer than that of the control group(4.20±1.95),P=0.000,which have statistically significant difference.The incidence of postpartum hemorrhage of the case group was 22.2% which is higher than that of the control group which is 11.8%,P=0.000,and the difference was statistically significant.Through Logistic regression analysis,forceps delivery,perineal cut and analgesia were the independent risk factors of postpartum urinary retention.ConclusionsThe occurrence of PUR can affect the recovery of postpartum,and prolonged the length of hospital stay and increased the risk of postpartum hemorrhage.Through single factor analysis,the risk factors of postpartum urinary retention including age,forceps delivery,analgesia,episiotomy,manual rotation of fetal head,the second stage of labor time.According to multivariate Logistic regression analysis,the independent risk factors of PUR: forceps,episiotomy,analgesia.
