Objective: The first part intended to summarize and explore the ancient and modern understanding,clinical application in the law of treatment of Blood Troubles,prescription meaning and Therapeutic significance of Cuhuang Powder which is invited by Tang Rongchuan.The aim of this part was to provide theoretical basis for the study on the therapeutical effect and mechanism of Cuhuang Powder.The second part intends to observe and research the curative effect and mechanism of Cuhuang Powder on Blood stasis and Bleeding of rats caused by High Molecular Weight Dextran(HMWD),it was aimed to provide experimental basis for clinical application.Methods: Guided by the theory of traditional Chinese Medicine and based on the Tang Rongchuan Treatise on Blood Troubles,the clinical and experimental data of traditional Chinese and Western medicine in recent ten years were analyzed.In animal experiment method,the model rats were induced by caudal vein injected of HMWD to causing the pathologicalchange of blood stasis and bleeding.The general condition of rats,mesenteric microcircu-lation,HE staining of lung tissue,hemorheology were tested in Experiment one.The level of NO?ET-1?VEGF?ES?TXB2?6-keto-PGF1??t-PA?PAI-1?GP?b/?a?GMP-140 and other cytokines in serum,platelet count,four items of blood coagulation were detected in part experiment.These were aimed at Studing on Microcirculation effect and mechanism of Cuhuang Powder.Results:The first part literature research discovered that: 1.Treatise on Blood Troubles was a monograph which discuss the blood trouble.Tang make much account of the relationship between blood stasis and hemorrhage.2.the Basic function of Cuhuang Powder was Xia-yu-zhi-xue.“Hua Yu Bu Dong Xue,Zhi Xue Bu Liu Yu”is its characteristic.Therefore it was the law of treatment of blood trouble in the modern clinical.The second part,1.Study on effect about microcirculation of Cuhuang Powder on Blood stasis and Bleeding of Rats caused by HMWD.(1)Objective index of blood stasis:(1)Hemorheology: Compared with the control group,the whole blood viscosity,capillary plasma viscosity and erythrocyte aggregation index of the model group increased(p<0.05).Compared with the model group,the above indexes of high-dose and low-dose group decreased(p<0.05).Compared with the model group,the above indexes of hemostatic group were not decreased,and the erythrocyte aggregation index increased(p<0.05).(2)Mesenteric microcirculation: Compared with the control group,the velocity of blood flow in the vessels of the mesenteric capillaries was significantly slowed down,and the diameter of blood vessels increased,especially in the venules,and the ratio of the arteries and veins was reduced(p<0.05).Compared with the model group,blood flow of high-dose and low-dose group increased,arteriovenous ratio increased(p<0.05).Compared with the model group,blood flow was slowed down in the rate of dicynone group(p<0.05).(3)Pathological examination of lung tissue: Compared with the control group,the lung tissue of rats in the model group was thickened,lung alveolar atrophy,part of the structure was destroyed,and the scattered red blood cells could be seen in the lung septum and the alveoli.The pathological changes of the lung tissue to improve,but the pulmonary septal thickness has not improved significantly in other groups.(2)Objective indicators of bleeding:(1)Mesenteric detection: comparedwith the control group,the number of blood vessels of rats in model group decreased,uneven thickness,vascular injury were observd.There are sporadic signs of old bleeding around the vessels.Compared with the model group,vascular condition was improved in high-dose and low-dose group,bleeding was significantly decreased.(2)Lung biopsy: compared with the control group,it was observed that red blood cells were seen in the pulmonary interstitial and alveolar cavity.The pathological score of lung was significantly higher(p<0.05).The score of other groups was reduction(p<0.05).(3)Mean gray value of lung tissue: In the model group,the percentage of the total area of lung tissue which is bleeding(or "ratio")increased,and the average gray level decreased.It showed that bleeding injury of Lung.Compared with the model group,ratio of other groups decreased,the average gray value increased(p<0.05).2.Study on mechanism of Cuhuang Powder on Blood stasis and Bleeding of rats caused by HMWD.(1)Endothelial function test:(1)Content of No and ET-1: Compared with the control group,the content of NO decreased and the content of ET-1increased in model group(p<0.05),and the proportion became imbalance.The content of NO increased,content of ET-1 decreased in high-dose and low-dose group(p<0.05).(2)Content of VEGF and ES: the content of ES decreased and the content of VEGF increased in model group(p<0.05).The content of ES increased,the content of VEGF decreased in other groups(p<0.05).(3)Content of TXB2 and6-keto-PGF1?: Compared with the control group,the content of TXB2 decreased and the content of 6-keto-PGF1?increased in model group(p<0.05).(2)Platelet function test:(1)platelet count: Compared with the control group,the platelet count of model group decreased(p<0.05).Compared with the model group,the count of high-dose group increased significantly(p<0.05).(2)Content of GMP-140 and GP?b/?a: Compared with the control group,the Content of GMP-140 and GP?b/?a decreased in model group(p<0.05).Comparedwith the model group,the Content of GMP-140 and GP?b/?a increased in other groups(p<0.05).(3)Fibrinolytic system function: Compared with the control group,the Content of t-PA in model group decreased(p<0.05).the Content of t-PA increased in other groups(p<0.05).(4)Coagulation system function: Compared with the control group,PT and FBG decreased,APTT and TT increased in model group(p<0.05).Compared with the model group,PT and FBG increased,APTT and TTdecreased in Cuhuang Powder group(p<0.05).Conclusion:1.The rat model of HWED was discovered the high blood viscosity,the slow blood flow velocity of the microcirculation,the blood deposition in the blood vessel,the wide interval of the lung tissue which were blood stasis change.Furthermore,Mesenteric microvascular hemorrhage and abnormal red blood cells in Lung biopsy was visible which were bleeding changes.So this model applied to study of Chinese medicine which was used for removing blood stasis and stopping bleeding.2.The above situation has improved in Cuhuang Powder high-dose and low-dose group.It shows that it has a better therapeutic effect on pathological changes of blood stasis and bleeding.The action of ethamsylate had some limitations becouse the effect of stopping bleeding was better,but it was not obvious to blood stasis.3.The study of the mechanism of Cuhuang Powder showed that it might play an important role in reducing the blood viscosity,affecting the function of vascular endothelium and platelets.
