Objective:To compare the safety and efficacy of preoperative FOLFOX and direct surgical treatment for advanced gastric cancer with pyloric obstruction. Methods : The clinical data of 40 advanced gastric cancer patients with helicobacter pylori admitted to the first affiliated hospital of Nanchang University from June 2013 to January 2017,To evaluate the clinical efficacy of preoperative FOLFOX chemotherapy, reoperation and direct surgery in patients with locally advanced pyloric obstruction, they were nonrandomly divided into two groups: Preoperative chemotherapy group(20 cases)and direct operation group(n = 20).The safety and efficacy of the two groups were compared between the two groups. Results: The resection rate of primary lesions in the observation group was significantly higher than tha t in the control group(P < 0.05)and the incidence of myelosuppression and gastrointestinal reaction in the observation group was significantly higher than that in the control group(P < 0.05);Also we found that the two groups of patients reached the ratio of radical effect had significant difference(P<0.05)and they did not differ between the incidence of postoperative complications. Conclusion: Preoperative use of oxaliplatin combined with fluorouracil and leucovorin FOLFOX chemotherapy for patients with advanced pyloric obstruction type gastric cancer of perioperative safety and efficacy was better than direct surgical treatment patients.
