Clinical Value Of Endoscopic Nasojejunal Feeding Tube Placement For Patients With Pyloric Obstruction Caused By Gastric Antral Cancer

Objective:To evaluate the advantages of endoscopically nasojejunal feeding tube placement (ENFTP) by accessory channel and the clinical value of enteral tube feeding for patients with pyloric obstruction caused by gastric antral cancer.Method:50 cases with pyloric obstruction caused by gastric antral cancer treated with ENFTP were analyzed retrospectively. Among those,29 cases who received enteral nutrition in our hospital were selected for record of nutrition relating indices on baseline and after 1 and 2 weeks of nutritional support. The observing indices included hemoglobin, lymphocyte count, blood glucose, total serum protein, serum albumin, serum creatinine, blood cholesterol, blood triglyceride, serum calcium, body weight and rates of surgery or chemotherapy. Meanwhile 25 patients treated with TPN were selected as controls.Results:ENFTP success rate was 98%(49/50). After 1 week of nutritional support, all the indices in ENFTP group changed insignificantly (P>0.05). Hemoglobin level and body weight in TPN group decreased significantly (P<0.05), and serum creatitine decreased highly significantly (P<0.01). There were no significant differences in rates of surgery and chemotherapy between the two groups (P>0.05). After 2 weeks, all the indices but serum creatitine in ENFTP group increased. Especially, the changes in total serum protein and serum albumin had statistical significance (P<0.05, P<0.01).Conclusion:ENFTP by accessory channel is safe and effective for patients with pyloric obstruction caused by gastric antral cancer. ENFTP was better than TPN in maintaining or improving the nutrition status of patients with gastric cancer.
