ObjectiveTo explore the microsurgical treatment and curative effect of falcial meningiomas in central area.To introduce the strategy of treatment of cerebral falx meningiomas in central area,in order to further improve the total resection rate,prolong the interval of recurrence and reduce the postoperative complications.MethodsA retrospective analysis of the Second Affiliated Hospital of Nanchang University during the period from December 2014 to December 2016 the microsurgery treatment of central falcial meningioma in 37 cases,retrospective analysis the general conditions,clinical manifestations,imaging findings,the surgical approach,intraoperative tumor resection techniques,postoperative follow-up,therapeutic effect and complications after operation.ResultsThere were 21 cases of Simpson grade I resection(56.8%),Simpson grade II resection in 12 cases(32.4%),Simpson grade III resection in 4 cases(10.8%),Simpson grade IV resection removal in 0 cases,and no death case.After operation,1 cases of acute encephaloeele during surgery,decompressive craniectomy,21 cases of limb muscle strength improved after surgery,3 cases worsen,2 cases were improved after treatment,1 cases of unilateral limb insufficiency paralysis.5 cases were treated with radiation therapy within 1 months after operation.All patients were followed up for 6months to 3 years.2 cases recurred.ConclusionIn the central area,the cerebral falx meningioma is adjacent to the functional area and the peripheral blood vessels are complex.Preoperative imaging should be performed to select the appropriate surgical approach.The key to improve the curative effect and reduce the complication of the operation is the exposure of the tumor,the protection of the central vein and vein,the protection of the cerebral cortex and the treatment of the superior sagittal sinus.In the case of the adhesion of tumorand surrounding tissue,it is not necessary to pursue the complete resection of the tumor.
