Epidemiological Characteristics And Medical Expenses Research Of Varicella In Karamay

Background:Varicella is an acute,highly contagious viral disease caused by the initial infection of varicella-zoster virus.Humans are only infective host of varicella-zoster virus.Varicella can be infectious through droplets or aerosols transmission by contacting the varicella-zoster virus in the blister fluid.Once varicella cases appears in the susceptible population,it is difficult to prevent the emergence of the outbreak.Effective control of the outbreak of varicella has become one of the important issues that can not be ignored in the field of public health.Widespread vaccination is an effective measure to prevent varicella epidemic.Based on the annual incidence of varicella in Karamay and the incidence of public health emergencies of varicella,varicella has become one of the more prominent infectious diseases in the public health incident in schools(kindergartens).In order to improve the mechanism of existing immunization strategy in Karamay,to promote disease prevention and control and public health work,to ensure the healthy growth of infants and young children,to achieve the highest economic and social benefits,we carry out Varicella epidemiology and disease burden research work in Karamay.Objective:To understand the epidemiological characteristics of the varicella in Karamay and key population in the prevention and control of varicella.Through the analysis of the quality of monitoring data,the investigation of false negative rateof varicella and the medical expenses of patients with varicella treatment in the area,we estimate the medical expenses of varicella cases in Karamay,determine the priority allocation of limited health resources,and provide a strong basis for improving the mechanism of the immunization strategy of Karamay,thus expanding the coverage of the varicella vaccine and protecting the susceptible population.Methods:Using the Chinese disease prevention and control information system to collect the data of varicella epidemic in Karamay,describe the epidemiological distribution of varicella and the analysis of the timeliness,accuracy and completeness of monitoring data.Through the stratified random sampling method,implement onsite questionnaire survey in four kindergartens to investigate the negative false rate of varicella;Collect medical expenses information for cases of varicella cases in four hospitals in the area,and analyze the medical expenses of patients with varicella treatment.Results:A total of 5 639 cases of varicella were reported in Karamay from 2006 to 2016.Among them,there were obvious seasonal peaks in April to July and October to December.The incidence of varicella was regional difference,and that of primary and middle school students,child care institutions and scattered children was high incidence population of varicella.In 3-year monitoring data,the average timely reporting rate was 94.07%,duplicate card rate was 1.62%,the wrong card rate was 0.17%.There was a certain percentage of the cases in which there were missing non-essential items.The survey of 4 kindergartens,issued 950 questionnaires,collected 908 questionares,the effective rate was 95.6%.The vaccination rate of varicella was 79.19%,and the false negative rate of varicella was 21.43%.Based on analysis of the survey,varicella outpatient per capita cost of 196.48 RMB;varicella inpatient per capita cost of 3,145.76 RMB.The total income was 3,314,214.27 RMB.Conclusions:The epidemiological characteristics of varicella in Karamay from 2013 to 2015 are basically same as those of other places.The timely reporting rate of Varicellain infectious reported network is decline by years.There are still a small number of errors and duplicated records in the network reporting varicella data,the health administrative departmentsshould strengthen the administration of varicella epidemic monitoring.The false negative rate of varicella is high,which affected the accuracy of infectious disease surveillance data.The survey is a retrospective study,there is a certain degree of recall bias,the results also have some limitations.Through the per capita varicella medical expenses and the cost of per capita varicella inoculation,with the increase in the number of vaccination of varicella vaccine,indicating that the vaccine acceptance of residents has been increasing,continued to implement the expansion of varicella vaccination program is scientific,feasible.
