Objectives: To explore the new method,Colorectal Epithelial Cell FH Test Kit,could be used in colorectal cancer screening,thus a preliminary evaluation of the clinical significance in colorectal cancer early screening.Method: The patients who visited Affiliated Hospital of Sichuan Medical College Annorectal and Gastrointestinal surgery and Gastroenterology dept were detected by the electrical colonoscope,even biopsy,from August 2016 to January 2017.Then randomly selected 102 cases which divided into three groups.The first group was 34 participants were diagnosed with colorectal cancer.The second group was 34 participants were diagnosed with precancerous lesions(include colorectal adenomas).The third group was 34 participants were normal people.The three group participants were performed FH test and fecal occult blood test and carcinoembryonic antigen(CEA)test.Observing and recording the results,then importing to SPSS 13.0 software for statistical analysis and then calculating the three technologies' s sensitivity,specificity,false negative,false positive rate,positive likelihood ratio,negative likelihood ratio and so on,verify the clinical application value of the new technologies for colorectal cancer screening.Result:(1)Through FH test and fecal occult blood test and carcinoembryonic antigen(CEA)test,the positive rate of the experimental group 1 was respectively 79.41%,94.12%,44.12%.The experimental group 2was respectively 52.94%,23.53%,8.82%.The control group was respectively11.76%,8.82%,2.94%.To compare the three screening ways of thedifferences in experimental the experimental group 1 and the experimental group 2,the differences between FH test,fecal occult blood test and CEA test were statistical significance(the experimental group 1:2? =22.546,P=0.000,the experimental group 2:2? =16.863,P=0.000,so P<0.05),while it was no statistical significance in the control group(2? =1.899,P=0.387,P>0.05).(2)The positive rate of the between Colorectal precancerous lesion group and cancer group were respectively 66.18%,58.82%,26.47%,so it was statistical significance(2? =24.277,P=0.000,P<0.05).what's more,it was statistical significance between FH test and CEA test(P<0.05),the similarly fecal occult blood test and CEA test(P<0.05).However,FH test and fecal occult blood test,the difference was not statistically significant(P>0.05).(3)In the study,the sensitivity of FH test,fecal occult blood test,CEA test were 66.18%,58.82%,26.47%;Specificity were 88.24%,91.18%,97.06%;False negative were33.82%,41.18%,73.53%;False positive rate were 11.76%,8.82%,2.94%;Positive likelihood ratio were 5.63,6.67,9.00;Negative likelihood ratio were0.38,0.45,0.76;Diagnostic index were 154.41%,150.00%,123.53%;Crude accuracy were 73.53%,69.61%,50.00%;Positive predictive value were91.84%,93.02%,94.74%;Negative predictive value were 56.60%,52.54%,39.76%.(4)In joint tests,the sensitivity and the false positive ratewere higher than the other single test.the false negative and the specificity were lower.For example,FH test and fecal occult blood test combined with CEA test,the sensitivity was highest and the false negative lowerest.Conclusion: Colorectal Epithelial Cell FH detection is a new method of colorectal cancer screening.Through this experiment,the colorectal epithelial cell FH detection positive cases showed an increasing trend in the normal people,colorectal precancerous lesion people and rectal cancer group.Colorectal Epithelial Cell FH Test Kit,characterized as simple,economical,rapid and efficient,has a high clinical application value,and the combination of the three methods helps to improve the sensitivity.What's more,company should improve the product,for example the sampler and specific color and so on.
