Objective:By constracting subjects with detecting FH in rectal epithelial leachate,colonoscopy and fecal occult blood results,to verify the relation between existing FH Test Kit For Stability Of Rectal Mucous Membrane Cell and early screening of colorectal.Method: 80 cases of anorectal outpatient department of Chengdu anorectal hospital were collected in accordance with the inclusion criteria.And they were randomly divided into two groups,the experimental group and the control group,40 cases in the experimental group and the control group.According to the control,single blind principle,the experimental group underwent colonoscopy and FH detection,the control group underwent colonoscopy and fecal occult blood test.Through the statistics of the results of the two groups,using statistical software SPSS21.0 to analyze the data of the statistical results,the sensitivity,accuracy and to compare and evaluate the FH Test Kit For Stability Of Rectal Mucous Membrane Cell’s and the fecal occult blood test’s sensitivity,accuracy.Results: there were 40 subjects in the test group,of which 18 cases were positive for FH test,and the positive rate was 45%.18 cases of positive samples,according to the follow-up colonoscopy and pathology examination confirmed the diagnosis of tubular adenoma in 9 cases,4 cases of inflammatory polyps,1 cases of proctitis,1cases of ulcerative colitis,3 cases of colonoscopy showed no obvious abnormalities.Of the patients with negative enteroscopy,1 had a history of gastric malignant tumor and underwent radical gastrectomy 6 years ago.The FH test was negative in 22 cases,of which 3 cases were diagnosed as tubular adenoma and 1 were 1+ years after radical resection of rectal cancer.In the experimental group,40 patients had no colorectal cancer,the sensitivity of colorectal polyps was 81.25%,the positive predictive valuewas 72.22%,the sensitivity of precancerous lesions was 83.33%,and the sensitivity was 83.33%.There were 40 subjects in the control group,of which 16 cases were fecal occult blood test positive,and the positive rate was 40%.Positive subjects according to follow-up colonoscopy and pathology examination confirmed the diagnosis showed: 4cases of tubular adenoma,2 cases of inflammatory polyps,2 cases of proctitis,1 cases of ulcerative colitis,1 cases of rectal cancer,2 cases of mixed hemorrhoids,anal fissure in 2 cases,2 cases had no obvious abnormalities in colonoscopy.The fecal occult blood test test was negative in 24 subjects,and 4 of them were diagnosed as tubular adenomas.Among the 40 patients in the trial group,1 cases were rectal adenocarcinoma patients,the Dukes stage was A2 stage,accounting for 2.5% of the control group,the positive predictive rate of colorectal polyps was 37.50%,the positive predictive rate of colorectal cancer precancerous lesions was 56.25%,and the positive predictive rate of colorectal cancer was 6.25%.The sensitivity of the experimental group and control group in colorectal polyps were compared by chi square test,X x2=1.3026,P=0.2537 > 0.05,the difference was not statistically significant;in the positive predictive rate,the value of X2=4.1423,P=0.0418 < 0.05,FH is better than the detection of fecal occult blood test.In the detection of colorectal cancer,there was no significant difference in sensitivity and positive predictive rate(chi square value x2=0.4219,P=0.5160 > 0.05;chi square value x2=1.8304,P=0.1761 > 0.05).In the test of colorectal cancer,because of the small number of cases the sensitivity causes chi square to test,the positive predictive rate of the two groups,the value of X2 =1.1591,P= 0.2817>0.05,the difference was not statistically significant.Conclusion: the FH Test Kit For Stability Of Rectal Mucous Membrane Cell for detection of colorectal polyps and its positive predictive rate was significantly higher than that of fecal occult blood test,in test node advantages of rectal cancer and precancerous lesions screening on fecal occult blood test is not feasible.The FH TestKit For Stability Of Rectal Mucous Membrane Cell needs to be improved to increase the sensitivity and positive rate of screening.
