Objective : To investigate the effect of FICE(Fuji intelligent chromoendoscopy,FICE)in the detection of central lung cancer,and to select the optimal channel setting combination to explore the diagnostic value of central bronchogenic carcinoma.Methods : From January 2016 to October 2016 by bronchoscopy in Chongqing Cancer Hospital,data intact,146 cases of patients with clear pathological diagnosis respectively.Using WLB(white light bronchoscopy,WLB)and FICE observation.FICE 10 channel settings respectively on the surface structure and morphology of vascular lesions were scored,choose the best channel setting,and forceps biopsy,pathological results as compared to the final study on the gold standard.Results:In 146 patients,the final pathology results to judge the malignantcases in 116 cases,30 cases of benign pathological.116 cases of malignant cases in diagnosis of 56 cases of squamous cell carcinoma,18 cases of adenocarcinoma,small cell undifferentiated carcinoma in 35 cases,3 cases of adenosquamous carcinoma,4 cases of pathological types of cancer is not clear.Endoscopic images differed significantly among 10 channels setting(P<0.01),Channel 1(R=550(2)? G=500(4)? B=470(4),[nm](gain))and Channel 8(R=540(2)?G=505(4)?B=420(5),[nm](gain))were ideal for the observation of the surface morphology and vascular texture.The coincidence rate was compared with that of pathology.The coincidence rate of the lesions was 88.4%,sensitivity was 92.2%,and 73.3%,specificity.Positive predictive value and negative predictive value were 93% and 71%.The detection rate of FICE combined with white light endoscopy was 96.6%,compared with that of single white endoscope,the detection rate was significantly different(P<0.01).In 17patients(14.7%),FICE revealed greater tumor extent than WLB alone.And tumor margins were >1cm wider on FICE examination than on WLB.In 12patients(10.3%),that finding influenced further therapeutic course.Conclusion : FICE technology has the advantages of simple operation,intuitive image.FICE Channel 1(R=550(2)?G=500(4)?B=470(4),[nm](gain))and Channel 8(R=540(2)?G=505(4)?B=420(5),[nm](gain))were more ideal for the observation of the surface structure and superficial mucosal capillary morphology of the central lung cancer.Combined with WLB can improve the accuracy of diagnosis of central lung cancer,effectively guide the targetedbiopsy,and it could also aid more accurate local staging of lung cancer,worthy of clinical application.
