Objective: To investigate the changes of the serum thymidine kinase1 levels in patients with non-small cell lung cancer after treatment(operation,chemotherapy)and its clinical signigicance.Methods: A total of 137 patients with NSCLC(lung cancer group)and 41 patients with benign disease(benign lesion group)were enrolled from July 2015 to December 2016 in our hospital,29 healthy subjects were included in the control group,compared the levels of serum thymidine kinase 1 with each group,and to analyze the correlation with clinical characteristics.After the radical resection of patients with recurrence or metastasis of single factor and multivariate factor analysis;and collect other tumor markers,such as CEA,CA125,CYFRA21-1,SCC for diagnostic tests.Results: The levels of sTK1(3.05 ± 1.97)in patients with non-small cell lung cancer were significantly higher than those in benign lesion group(1.54 ± 1.38)and healthy control group(0.77 ± 0.49),P <0.001.The level of sTK1 was correlated with the degree of differentiation,tumor size,TNM staging and lymph node metastasis.The difference was more obvious in the degree of tumor differentiation,but not with other factors such as sex,age,smoking history and tumor pathological type P>0.05).In the case of lymph node metastasis,N3 and N0 and N1-2 were statistically significant difference between the two groups,while N0 and N1-2,P = 0.216,no significant difference.In the combined diagnostic test,the sensitivity of individual sTK1 was 70.9%,the specificity was 88.1%,and combined other tumor markers the sensitivity was significantly improved,which was higher than that of alone.In the patients undergoing surgery,the level of sTK1 was significantly decreased(p <0.05)at 1month postoperatively.The patients with recurrence and metastasis after operation were analyzed by univariate analysis of clinical and pathological parameters.It was found that high sTK1(P <0.05).Furthermore,multivariate analysis of COX regression model showed that the level of high sTK1 and lymph node metastasis were independent influencing factors.The level of TNM staging was correlated with recurrence and metastasis(p <0.05).In patients with chemotherapy,chemotherapy 2 to 4 courses of patients after sTK1 levels weresignificantly reduced(p <0.05).Conclusion:1,The expression level of sTK1 was correlated with tumor size,differentiation,staging and lymph node metastasis;2,STK1 combined with other tumor markers CEA,CA125,CYFRA21-1 and SCC can improve the sensitivity and specificity of diagnosis of NSCLC;3,For postoperative and chemotherapy patients,monitoring sTK1 levels and other risk factors may be one of the indicators of postoperative recurrence prediction and efficacy.
