The Curative Effect Analysis On The Treatment Of Long Comminuted Fracture Of Proximal Femoral By Long PFNA And PTGN

Objective:To compare the clinical effect surgical with case of prolonged third generation of Gamma nail(pTGN)and long proximal femoral nail antirotation(long PFNA)on the treatment of long comminuted fracture of proximal femoral,for long comminuted fracture of proximal femoral patients provide a reference for the choice of internal fixation.Method:The review of the research Guangxi medical university third affiliated hospital orthopedic from June 2014 to May 2016 were 65 cases of long comminuted fracture of proximal femoral.The patients were divided into pTGN group(33 cases)and long PFNA group(32 cases)according to surgical methods.Through the two groups of patients wth operation time,intraoperative blood loss,Tip apex distance(TAD),length of hospital stay,postoperative complications,fracture healing time and postoperative hip function recovery degree and so on aspect to compare their differences.SPSS22.0 is used to analyze the data.Results:Long PFNA and pTGN group operation time,intraoperative blood loss were 122.15±17.65min and 132.58±20.50min?250.44±20.99ml and 280.18±16.58ml,the comparison between the two groups,the difference was statistically significant(P<0.05).Singh index level 3 or less patients,Long PFNA and pTGN group TAD at postoperative months 6 were 20.66±4.16mm and 23.96±3.56mm,the difference was statistically significant(P<0.05).In the two groups in length of hospital stay,fracture clinical healing time,postoperative complications,the Harris score at postoperative months 6 and 12 in comparison,there was no statistically significant differenc(P<0.05).Conclusion:Comapared with pTGN,long PFNA treatment of osteoporosis patients withlong comminuted fracture of proximal femoral,can give full play to the proximal screw blade compresion cancellous bone,and the advantages of the rotational resistance,resistance to cut.Comapared with pTGN,long PFNA with more convenient operation,short operation time and less intraoperative blood loss.pTGN is more suitable for young patients with excellent bone quality with long comminuted fracture of proximal femoral,can give full play to the proximal tension screw pressurizing advantage.Long PFNA and pTGN group patients'length of hospital stay is short,can early ambulation,fewer complications,the hip joint function recovery,Both of them are under the treatment of long comminuted fracture of proximal femoral is reliable and effective internal fixation.Acocording to the actual circumstances of the patients,Within a reasonable choice fixture is a key to the success of long comminuted fracture of proximal femoral.
