| Objective:Meta-analysis comparing the proximal femur nail antirotation versus hemiarthroplasty with the clinical effect of intertrochanteric fractures in elderly.Methods:PubMed,MEDLINE,EMBASE,Cochrane,OVID,CNKI,VIP,Wanfang,CBM database were compared between the treatment of proximal femur nail antirotation versus hemiarthroplasty of old femoral intertrochanteric fractures Literature from June2004 to November 2017,Results The evaluation indexes included operation time,intraoperative blood loss,hospitalization days,Harris score,Implant-related complications、total postoperative complications,postoperative 1-year mortality,RevMan5.3 software analysis of data,P <0.05 was statistically significant.Results:7 papers(3 Chinese and 4 English)were finally included.Proximal femoral nail anti-rotation group have a shorter operation time[WMD=-12.53,95%CI(-14.36,-10.70),P<0.05]、Less intraoperative blood transfusion[WMD=-189.88,95%CI(-317.84,-61.91),P<0.05]、Lower1-year mortality [OR=0.53,95%CI(0.34,0.83),P<0.05]compared with hemiarthroplasty,There was no statistical difference between the two groups in hospital days[WMD=2.57,95%CI(-1.79,6.94),P=0.25] 、 Postoperative Harris hip score[WMD=-0.50,95%CI(-1.54,0.54),P=0.35] 、Implant-related complications[OR=1.03,95%CI(0.57,1.88),P=0.92] and total postoperative complications [OR=1.90,95%CI(0.74,4.91),P=0.18].Conclusion:In the treatment of patients with intertrochanteric fractures of the elderly,We recommend preferential selection of proximal femoral nail anti-rotation. |
