Clinical Analysis Of Cryptogenic Organizing Pneumonia

Objective:To improve the knowledge of cryptogenic organizing pneumonia(COP),and reduce misdiagnosis and mistreatment,as well as the mental pressure and economic burden of patients and their families.Methods:One case of lung disease from Shandong University of Traditional Chinese Medicine Affiliated Hospital of COP cases,and China cases to retrieve a complete database from January 2006 to December 2016 were reported in the literature of our country clear diagnosis,The mode of onset,clinical manifestation,auxiliary examination,diagnosis,treatment and prognosis of the disease model results were retrospectively analyzed.Results: A total of 30 patients with COP were collected,including 16 males,4 of them is smoking,The age of the patients ranged from 22 to 81 years old.The most common symptom was cough(25 cases),Laboratory examination of ESR and C-reactive protein C increased in different degrees,Pulmonary function was mainly restricted by restrictive ventilation dysfunction and decreased diffusion function,In 26 cases of Chest CT showed multiple patchy or patchy consolidation or high density shadow in the bilateral lung,in which there were signs of inflation in the bronchus pleural effusion in 7 patients.26 cases were diagnosed of pneumonia or lung infection at the beginning,rate of misdiagnosis up to 100%.30 patients were treated antibiotic treatment in different treatment for the first time,then Glucocorticoid therapy after diagnosis,the effect is good,4 patients had recurrence,including 1 case of recurrenc,recurrence were related to hormone reduction or discontinuation.Conclusions:There was no significant difference between male and female patients with COP,and the age of onset was 40 years old and-60 years old.There was no specificity in clinical manifestation and auxiliary examination.The diagnosis is unique,advocate the clinical radiological and pathological diagnosis(CRP)multi disciplinary cooperation mode,but in the case of the disease may be suspected of clinical and imaging,diagnosis of hormone therapy on the diagnosis of clinical significance,easy misdiagnosis,in anti infection treatment effect of this disease may be considered without rear.The treatment of drugs is Glucocorticoid,Macrocyclics lipid drug treatment is also effective,some patients can use it as replacement way,recurrence is related to not standardized application of hormone(reduction or withdrawal,etc).
