Objective:Helicobacter pylori(H.pylori)has been studied extensively since it was discovered in 1983.In addition to the occurrence and development of some digestive system diseases such as chronic gastritis,gastric ulcer,gastric cancer and mucosa-associated lymphoid tissue(MALT).It may be relate to other system diseases such as cerebral infarction,idiopathic thrombocytopenic puroura(ITP),unexplained iron-deficiency anemia and so on.And it even associate with coronary heart disease,urticarial and rosacea.There are many kinds of diseases in Department of General Internal Medicine-including diseases of many systems.And most of the patients are the elderly,but the H.pylori infection situation in them has not yet been reported.Based on the above problems,1652 casesin the Department of General Internal Medicine of Kunming General Hospital from July 1,2013 to September 30,2015 were analyzed,in order to investigate the H.pylori infection status and related factors in them,especially in the elderly.And another aim was to explore the H.pylori infection status in cerebral infarction patients.Methods:H.pyloriwas tested by 13C-urea breath test(13C-UBT)in 1652 cases in our hospital from July 1,2013 to September 30,2015.Detection method:In the morning,the patients who were fastingbreathed into the bottom bag for 3 to 5 seconds.The bag was tightened rapidly.And then,one 13C urea capsule was taken with about 20ml cold boiled water.25 minutes later,patients were asked to breathe into another bag.The samples were analyzed by the specialized instrument automaticly.The expiration value after taking 13C urea capsules 25 min later minus the expiration value before is delta over baseline(DOB)value.Outcome judgment:DOB waspositive when it was higher than or equal to 4,and less than 4 is negative.The information of gender,age,prevalence,H.pylori infection of all cases were analyzed.And the diagnosis of the disease was consistent with the corresponding diagnostic criteria,with the corresponding imaging or laboratory evidence.The first diagnosis was taken if there were more than one diagnosis for each patient.The patients with cerebral infarction were selected,The H.pylori infection status and its related factors were analyzed.Results:1.The H.pyloricurrent infection rate of all cases was 35.8%(591/1652).And it was 37.1%(308/830)in male group and 34.4%(283/822)in female group.There was no significant difference between the both groups(P>0.05).2.From the age distribution,H.pylori current infection rate in middle-aged group(39.2%,155/395)and younger elderly group(38.1%,266/699)were significantly higher than in youth group(27.4%,48/175)(P<0.01,respectively)and elderly group(31.9%,122/383)(P<0.05,respectively).H.pylori infection rate was gradually increased with age,reached a peak in the middle-aged group,and then decreased.3.From the system distribution,H.pylori infection rate in different systems from high to low was nervous system 41.3%(321/778),circulatory system 37.7%(113/300),respiratory system 29.7%(54/182),digestive system 26.3%(67/255)and other systems 23.3%(36/137).4.From the disease distribution,the H.pylori current infection rate in different diseases from high to low was coronary atherosclerotic heart disease42.9%(42/98),cerebrovascular disease41.7%(302/724),acute gastroenteritis38.5%(10/26),gastrointestinal bleeding37.5%(3/8),vertigo syndrome35.3%(6/17),bronchitis35.0%(28/80),primary hypertension33.2%(59/178)and functional dyspepsia32.0%(8/25)..5.The H.pylori infection rate in cerebral infarction group was 40.54%(257/634),which was significantly higher than control group(33.93%)(P<0.05).There was no significant difference among all age groups in cerebral infarction cases(P>0.05).And there was no significant difference between different times of cerebral infarction attack(P>0.05).Conclusions:1.H.pylori current infection rate in inpatients in department of generalinternal medicine is high,especially in the middle-aged group and younger elderly group.2.The H.pylori current infection rate in inpatients in department of generalinternalmedicine has no significant differences in gender.3.H.pylori infection rate among different diseases or systems is various.And each has its different clinical significance.4.H.pylori infection rate in patients with cerebral infarction is higher than the control group and its has significant difference.So the infection of H.pylori may be associated with the occurrence of cerebral infarction.There is no significant difference among all age groups in cerebral infarction cases.And there is no significant difference between times of cerebral infarction attacks.
