Objective:Since Marshall and Warren found Helicobacter pylori(HP),more and more people do research on HP.But the report of HP current infection status in gastrointestinal inpatients is rare.It may lead to that many HP current infection inpatients are overlooked by clinicians,HP's drug resistance is enhanced,and,it is difficult to be eradicated.Meanwhile,HP current infection is associated with many digestive system diseases.It is not only associated with gastrointestinal disease and extragastric disease,but also has a connection with outside the digestive system diseases.This study is based on gastrointestinal inpatients of Kunming General Hospital of Chengdu Military Command.The aims of this study are to investigate the current infection status of HP in inpatients in department of gastroenterology,and to analyze the related factors.Methods:HP was tested by 13C-Urea Breath Test in 3684 gastrointestinal inpatients of our hospital,which was selected from October 2011 to May 2015.And the HP current infection status was analyzed in different groups of gender,age and diseases.Results:1.The total HP current infection rate of all cases was 23.0%(848/3684).And it was 23.5%in male group and 22.4%in female group.There was no significant difference between the both groups(P>0.05).2.From the age distribution,HP current infection rate in middle-aged group(26.3%,322/1223)was significantly higher than that in youth group(19.4%,160/664)(P<0.01)and elderly group(23.1%,378/1259)(P<0.05).And it was significantly higher in elderly group than that in youth group(P<0.05).It was gradually increased with age,reached a peak in the middle-aged group,and then decreased.3.From the gender distribution,HP current infection rate was 28.7%(187/650)in male middle-aged group and 23.6%(135/573)in female middle-aged group.It was significantly higher in male middle-aged group than that in female middle-aged group(P<0.05).But there was no significant difference in male and female between in youth group and in elderly group(P>0.05).4.From different organ distribution,HP current infection rate in upper gastrointestinal diseases,low digestive tract diseases,hepatobiliary diseases,pancreatic diseases and enterocoelia diseases was 25.5%,20.3%,19.0%,10.4%and 3.0%,respectively.There was significant difference between the five groups(P<0.01).And it was the highest in upper gastrointestinal diseases(p<0.05).5.From common gastrointestinal disease distribution,the highest HP infection rates of five kinds of diseases were peptic ulcer(36.7%,166/452),gastric malignant tumor(30.6%,19/32),chronic gastritis(26.1%,354/1354),colonic malignant tumor(25.9%,14/54)and colon polyps(25%,30/120).However,the lowest infection rate of the disease was gastroesophageal reflux disease.Conclusions:1.HP current infection rate in gastrointestinal inpatients is high,especially in the middle-aged group.It is gradually increased with age,reached a peak in the middle-aged group,and then decreased.2.Across different age-groups and gender,HP current infection rate in male middle-aged group is significantly higher than in female middle-aged group.3.HP current infection rate in upper gastrointestinal diseases is significantly higher than that in low digestive tract diseases.And in low digestive tract diseases,the HP infection rate in colonic malignant tumor and colon polyps is gradually increased.It may be increased the risk of colonic malignant tumor and colon polyps.It is worthy of further attention.4.HP infection may be one of the important causes of hepatobiliary diseases and pancreatic diseases.It is worthy of further attention.5.HP is main pathogenic bacteria on peptic ulcer,gastric malignant tumor and chronic gastritis.HP has a protective effect of gastroesophageal reflux disease.6.There are different current infection rates among different diseases.It has different clinical significance.
